Opis:Izhodišča: Parkinsonova bolezen je nevrološka motnja, ki je eden najpogostejših vzrokov za nastanek in razvoj demence. Obravnava pacienta s to boleznijo zahteva individualiziran terapevtski načrt, ki je oblikovan v sodelovanju z multidisciplinarnim timom. Temelj tega je tudi teorija Doroteje Orem, katere koncepti zagovarjajo spodbujanje samostojnosti pacienta s Parkinsonovo boleznijo. Namen raziskave je preučiti načine aplikacije teorije Doroteje Orem pri zdravstveni obravnavi pacienta s Parkinsonovo boleznijo. Metode: V magistrskem delu smo uporabili kvalitativni raziskovalni pristop z deskriptivno metodo dela. V teoretičnem delu zaključnega dela je bil narejen pregled strokovne in znanstvene literature domačih in mednarodnih avtorjev. Podatke smo pridobili v naslednjih podatkovnih bazah: PubMed, CINAHL, Google Učenjak in SprinerOpen. Primarne podatke za empirični del smo pridobili s tehniko intervjuvanja. Ta je bila izvedena s polstrukturiranim vprašalnikom, ki smo ga oblikovali na podlagi pregleda literature. Rezultati: V raziskavi, v kateri je sodelovalo enajst intervjuvancev, smo ugotovili, da izvajalci zdravstvene nege ocenjujejo konceptualni model samooskrbe Doroteje Orem kot ustreznega za zdravstveno obravnavo pacientov s Parkinsonovo boleznijo. Ta naj bi še posebej spodbujal samostojnost pacienta pri opravljanju vsakodnevnih aktivnosti, zato je aplikacija teorije Doroteje Orem še posebej uspešna pri pacientih s Parkinsonovo boleznijo. Njena aplikacija se izvaja pri vseh temeljnih življenjskih aktivnostih, sami izidi pa so večinoma uspešni – odvisno od napredka bolezni. Ugotovljeno je bilo, da se večina izvajalcev zdravstvene nege prepoznava kot nosilci oz. izvajalci novih zdravstvenih praks, ki slonijo na teoriji Doroteje Orem, a je spodbudnih okolij v Sloveniji premalo. Razprava: Pacient s Parkinsonovo boleznijo zahteva kompleksno obravnavo. Pomembno je, da je ta deležen obravnave interdisciplinarnega in multidisciplinarnega tima, ki se nemudoma odzove z individualiziranim terapevtskim načrtom. Sodelovanje v takih timih je zahtevno, še posebej za izvajalce zdravstvene nege, ki kot koordinatorji obravnave sodelujejo tudi s svojci in pacienti. Zato je pomembno, da so ti usposobljeni, imajo mehke veščine, so kritični in poznajo na dokazih podprto prakso. V prihodnje bi bilo mogoče raziskavo razširiti, in sicer v razvoj modela zdravstvene obravnave pacienta s Parkinsonovo boleznijo, ki temelji na teoretičnih konceptih Doroteje Orem.

Naslov:Application of Dotothea Orem theoriy on patient with Parkinsons disease
Opis:Introduction: Parkinson's disease is a neurological disorder that is one of the most common causes of dementia. The management of a patient with this disease requires an individualised treatment plan, developed in collaboration with a multidisciplinary team. This is also based on the theory of Dorothea Orem, whose concepts advocate promoting the independence of the Parkinson's patient. The aim of this study is to examine how Dorothea Orem's theory can be applied to the medical management of the Parkinson's patient. Methods: We used two research methods - descriptive and qualitative. In the theoretical part of the thesis, a review of the professional and scientific literature of national and international authors was carried out. PubMed, CINAHL, Google Scholar and SprinerOpen. For the purposes of the empirical work, qualitative research was carried out using an interview technique. The latter was carried out using a semi-structured questionnaire, which was designed on the basis of a literature review. Results: In a survey of 11 interviewees, we found that healthcare providers perceived Dorothy Orem's conceptual model of self-care as appropriate for the healthcare management of Parkinson's patients. It is thought to promote patient independence in activities of daily living, and therefore the application of Dorothy Orem's theory is particularly successful for patients with Parkinson's disease. Its application is applied to all basic activities of life, and the outcomes themselves are mostly successful - depending on the progress of the disease itself. It was found that most health care providers recognise themselves as promoters or implementers of new health care practices based on Dorothea Orem's theory, but there are not enough supportive environments in Slovenia. Discussion: The Parkinson's patient requires complex management. It is important that he/she receives treatment from an inter- or multidisciplinary team that responds immediately with an individualised treatment plan. Working in such teams is challenging, especially for health care providers who also work as coordinators with relatives and patients. It is therefore important that they are trained, have soft skills, are critical and are familiar with evidence-based practice. In the future, it is worthwhile to extend the research The research could be extended to develop a model of Parkinson's care based on Dorothea Orem's theoretical concepts.
