Title:UČINKOVITOST FIZIOTERAPEVTSKE OBRAVNAVE PACIENTOV Z NATRGANINO ROTATORNE MANŠETE

Abstract:Ramenski sklep je zaradi svoje izjemne gibljivosti podvržen številnim poškodbam in okvaram. Ena izmed najpogostejših je natrganina mišic rotatorne manšete (RM) in njena incidenca se s starostjo povečuje. Značilni simptomi so bolečina v ramenskem sklepu ter zmanjšan obseg gibljivosti in mišična jakost. Natrganina RM je lahko simptomatska ali asimptomatska. Pravilna diagnoza je ključnega pomena za načrtovanje ustreznega zdravljenja. Pri postavljanju diagnoze so nam v pomoč klinični testi, vendar so slikovne metode MRI in MRA najbolj zanesljive. Možnost zdravljenja natrganin RM je konzervativno ali operativno. Za ustrezno določanje možnosti zdravljenja moramo upoštevati splošno zdravstveno stanje posameznika, funkcionalne zahteve, resnost natrganine in temeljito poznati morfološke lastnosti posameznika. Konzervativno zdravljenje je učinkovito in se priporoča pri starešjih bolnikih, saj imajo nizke funkcionalne zahteve. Nasprotno velja za operativno zdravljenje, ki je bolj priporočljivo za mlajše bolnike z nizkimi funkcionalnimi zahtevami. Raziskave so pokazale, da ima konzervativno zdravljenje primerljive učinke kot kirurško zdravljenje. Fizioterapevtske metode kot so vadbena terapija, manualna terapija, ultrazvočna terapija, akupunktura v kombinaciji s korekcijskimi vajami, specifično vadbo za aktivacijo im moč lopatičnih mišic ter vaje za gibljivost in raztezanje ramenskih mišic. Po menju avtorjev je najpogostejši kriterij za odločanje o kirurškem posegu delna ali popolna izguba funkcije in mišične moči v ramenskem sklepu. Kljub temu so indikacije za kirurško zdravljenje pomanjkljive in neenotne.

Title:Effectivness of physiotherapy treatment in patients with rotator cuff tear
Abstract:The shoulder joint is exposed to many injuries and disorders due to its extreme mobility. One of the most common is a rotator cuff (RC) muscle tear, and its incidence increases with age. Typical symptoms are pain in the shoulder joint and reduced range of movement and muscle strength. RM strain can be symptomatic or asymptomatic. A correct diagnosis is crucial to plan appropriate treatment. Clinical tests are helpful in making the diagnosis, but MRI and MRA are the most reliable imaging methods. The treatment option for ruptured RC is conservative or surgical. In order to properly determine the treatment options, we need to take into consideration the general health status of the individual, the functional requirements, the severity of the natrganina and a thorough knowledge of the morphological characteristics of the individual. Conservative treatment is effective and is recommended in elderly patients as they have low functional requirements. The opposite is true for operative treatment, which is more recommended for younger patients with low functional requirements. Research has shown that conservative treatment has comparable effects to surgical treatment. Physiotherapy methods such as exercise therapy, manual therapy, ultrasound therapy, acupuncture combined with corrective exercises, specific exercises to activate and strengthen the scapular muscles, and exercises for mobility and stretching of the shoulder muscles. According to the authors, the most common criteria for deciding on surgery is partial or complete dysfunction and muscle strength in the shoulder joint. However, the indications for surgical treatment are scarce and patchy.
