Title:Vpliv ekscentrične vadbe na bolečino pri odraslih s tendinopatijo ahilove tetive : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Teoretično izhodišče: Ahilova tendinopatija je stanje, za katero so značilni bolečina in otekanje tetive. Zdravljenje le-te velja za zelo počasen, dolgotrajen in velikokrat neuspešen proces. Namen: Namen diplomskega dela je bil na podlagi pregleda strokovne in znanstvene literature analizirati in odgovoriti na raziskovalna vprašanja. Zanimalo nas je, kako ekscentrična vadba vpliva na bolečino in funkcionalnost pri Ahilovi tendinopatiji, kakšen je vpliv ekscentrične vadbe na bolečino skozi čas, kakšna je uspešnost ekscentrične vadbe v primerjavi z drugimi fizioterapevtskimi pristopi na odpravljanje bolečine in katere so pomanjkljivosti Alfredsonovega protokola. Metoda: V diplomskem delu smo uporabili kvalitativno metodologijo. V teoretičnem delu smo po pregledu znanstvene in strokovne literature povzeli podatke z metodo opisovanja. V empiričnem delu smo po pregledu znanstvene in strokovne literature, ki smo jo izbrali s pomočjo diagrama PRIZMA, analizirali in predstavili 7 izbranih raziskav. Rezultati: Ugotovljeno je bilo, da ima ekscentrična vadba pozitivne dolgoročne učinke na bolečino in funkcionalnost. Po analizi pregledanih raziskav je znano, da do četrtega tedna ekscentrične vadbe skoraj ne vplivajo na bolečino. Pozitivni rezultati nastajajo po četrtem in se eksponentno izboljšujejo do dvanajstega tedna, kjer se nekoliko umirijo, vendar kljub temu napredujejo skozi čas. Razprava in zaključek: Za zdravljenje Ahilove tendinopatije je smiselno ekscentrično vadbo kombinirati z ostalimi fizioterapevtskimi pristopi, ki, kot je ugotovljeno, prinašajo kratkoročne učinke. Alfredsonov protokol se je izkazal kot uspešen, vendar nekoliko pomanjkljiv protokol, ki mu lahko pripišemo začetno stagnacijo pri zdravljenju Ahilove tendinopatije, zato bi bilo glede na posameznika potrebno prilagoditi število ponovitev, intenzivnost in bolje definirati obremenitev pri vadbi.

Abstract:Theoretical background: Achilles tendinopathy is a condition characterised by pain and swelling of the tendon. Its treatment is considered a very slow, lengthy and often unsuccessful process. Purpose: The aim of this thesis was to analyse and answer the research questions based on a review of the professional and scientific literature. We were interested in how eccentric exercise affects pain and functionality in Achilles tendinopathy, what is the impact of eccentric exercise on pain over time, what is the success rate of eccentric exercise compared to other physiotherapeutic approaches to pain management and what are the shortcomings of the Alfredson protocol. Method: In the diploma thesis we used a qualitative methodology. In the theoretical part, after reviewing the scientific and professional literature, we summarized the data using the descriptive method. In the empirical part, after reviewing the scientific and professional literature, we analysed and presented 7 selected studies using the PRIZMA diagram. Results: It was found that eccentric exercise has positive long-term effects on pain and functional capacity. After analysis of the reviewed studies, it is known that up to the fourth week, eccentric exercise has almost no effect on pain. Positive results emerge after the fourth week and improve exponentially up to the twelfth week, where they diminish somewhat but nevertheless progress over time. Discussion and conclusion: For the treatment of Achilles tendinopathy, it makes sense to combine eccentric exercise with other physiotherapy approaches that have been found to have short-term effects. The Alfredson protocol has proven to be a successful but somewhat flawed protocol, which may be responsible for the initial stagnation in the treatment of Achilles tendinopathy, and the number of repetitions, intensity and exercise load should be adjusted depending on the individual.
