Title:Fizioterapija pri utesnitvenem sindromu kolka : pregled literature

Abstract:Diplomsko delo obravnava vlogo fizioterapije (FT) kot ključnega pristopa pri zdravljenju utesnitvenega sindroma kolka (FAIS, angl. femoroacetabular impingement syndrome). Namen dela je, raziskati sodobne FT pristope pri zdravljenju FAIS in oceniti njihovo učinkovitost v primerjavi z drugimi konservativnimi metodami. FT se ne primerja z operativnimi posegi, ampak izključno z drugimi konservativnimi metodami. V empiričnem delu je raziskava osredotočena na štiri ključna vprašanja: (1) vlogo FT pri zdravljenju FAIS, (2) najpogosteje uporabljene postopke FT, (3) učinkovitost postopkov FT (4) primerjava FT z drugimi konservativnimi načini zdravljenja. Pregled strokovne literature kaže, da FT pomembno prispeva k lajšanju simptomov in izboljšanju funkcionalnosti kolčnega sklepa pri pacientih s FAIS. Najbolj pogosto se izvajajo kinezioterapevtske vaje. Še posebej učinkoviti so individualno prilagojeni vadbeni programi, ki vključujejo optimizacijo biomehanike, stabilizacijo trupa ter vadbo za moč in gibljivost, kar lajša bolečine in prispeva k rehabilitaciji. FT obravnava lahko odloži ali celo prepreči potrebo po operaciji. V raziskavah, ki so bile obravnavane v empiričnem delu, je manualna terapija pogosto kombinirana s kinezioterapijo. Prav tako lahko opornice za kolk zmanjšujejo bolečino in stabilizirajo sklep, kar olajša prehod na aktivne FT pristope, zlasti v akutnih fazah. Individualno prilagojeni FT programi, kombinirani z metodami, kot so intraartikularne injekcije in prilagoditve aktivnosti, so zelo učinkoviti. Kljub temu FT pri izrazitih morfoloških nepravilnostih, kot je cam utesnitev, ni vedno uspešna, kar kaže da so potrebne nadaljne raziskave dolgoročnih učinkov.

Abstract:This thesis discusses the role of physiotherapy (PT) as a key approach in the treatment of femoroacetabular impingement syndrome (FAIS). The aim of the thesis is to explore modern PT approaches in the treatment of FAIS and evaluate their effectiveness compared to other conservative methods. PT is not compared with surgical procedures but exclusively with other conservative methods. The empirical part of the research focuses on four key questions: (1) the role of PT in the treatment of FAIS, (2) the most commonly used procedures, (3) their effectiveness, and (4) a comparison with other conservative treatment methods. A review of the scientific literature shows that PT plays a crucial role in alleviating symptoms and improving hip joint function in patients with FAIS. The most commonly used exercises are kinesiotherapy-based. Particularly effective are individually tailored exercise programs that include optimizing biomechanics, trunk stabilization, and exercises for strength and flexibility, which contribute to pain relief and rehabilitation and can delay or even prevent the need for surgery. In the studies reviewed in the empirical part, manual therapy is often combined with kinesitherapy. Hip braces can also reduce pain and stabilize the joint, facilitating the transition to active PT approaches, especially during the acute phases. Individually tailored PT programs, combined with methods such as intra-articular injections and activity modifications, have proven to be highly effective. However, PT is not always successful in cases of pronounced morphological abnormalities, such as cam impingement, indicating the need for further research on long-term effects.
