Title:Vpliv zdravstvenih in demografskih dejavnikov na uspešnost fizioterapije pri kronični ledveni bolečini : doktorska disertacija študijskega programa tretje bolonjske stopnje Fizioterapija

Abstract:Kronična ledvena bolečina je kompleksen pojem, na katerega vpliva več dejavnikov: nevrobioloških, psiholoških in socioekonomskih. V pričujoči raziskavi smo preučevali vpliv bolečine, stresa, zaupanja v fizioterapevtsko obravnavo in kakovost življenja pacientov na uspešnost fizioterapije pri obravnavanem bolečinskem sindromu. V prvem delu disertacije smo z bibliometričnim pregledom bibliografije naredili poglobljen pregled znanstvene literature. Pri tem smo se osredotočili na analizo demografskih in zdravstvenih dejavnikov tveganja za nastanek in potek KLB pri delovno aktivnem prebivalstvu. V drugem delu smo predstavili metodologijo in rezultate raziskave. Pričujoča študija je predeksperimentalna raziskava s pristopom pred in po testiranju, opravljena na vzorcu 123 pacientov. Uporabljene so bile različne statistične metode: deskriptivna statistika, multipla linearna regresijska analiza in ANOVA. V tretjem delu študije smo pridobljene rezultate interpretirali glede na kritične zaključke pregleda do zdaj znane literature in pri tem odgovorili na zastavljeni raziskovalni vprašanji, in sicer: katera od obravnavanih spremenljivk in na kak način vpliva na uspešnost fizioterapije pri KLB. Zaradi tesne povezanosti obravnavanih spremenljivk smo interpretacijo poglobili z raziskovalnimi hipotezami. Ugotovili smo, da imata največji vpliv bolečina in stres, medtem ko kakovost življenja, zaupanje in demografski dejavniki kažejo šibke in neznačilne povezave. Študija združuje fizioterapevtski, javnozdravstveni in vedenjsko-kognitivni vidik aktualne problematike aktivnega delovnega prebivalstva, kar je bistvenega pomena za celostni pristop k obravnavi KLB. Z ugotovitvami prispevamo k razvijanju individualiziranega pristopa fizioterapevtske obravnave in optimizaciji fizioterapevtskih pristopov na podlagi psiho-telesnih značilnosti ranljive populacije pacientov s KLB. Študija vključuje tudi poudarek na zaupanju kot pomembnem psihosocialnem dejavniku KLB, kar predstavlja nov doprinos znanosti.

Abstract:Chronic low back pain (CLBP) is a complex concept influenced by several factors: neurobiological, psychological and socioeconomic. In the present study, we studied the impact of pain, stress, trust in physiotherapy treatment and quality of life of patients on the success of physiotherapy in the pain syndrome under consideration. In the first part of the dissertation, we conducted an in-depth review of the existing scientific literature using a bibliometric review of the bibliography. We focused on the analysis of demographic and health risk factors for the onset and course of CLBP in the working population. In the second part, we presented the methodology and results of the study. The present study is a pre-experimental study with a pre- and post-testing approach, conducted on a sample of 123 patients. Various statistical methods were used: descriptive statistics, multiple linear regression analysis and ANOVA. In the third part of the study, we interpreted the results obtained in relation to the critical conclusions of the review of the literature known so far, and in doing so, we answered the research questions posed, namely: which of the variables under consideration and in what way does it affect the success of physiotherapy in CLBP. Due to the close interrelationship of the variables under consideration, we deepened the interpretation with the help of research hypotheses. We found that pain and stress have the greatest impact, while quality of life, trust and demographic factors show weak and uncharacteristic connections. The study combines physiotherapy, public health and behavioral-cognitive aspects of the current problems of the active working population, which is of essential importance for a holistic approach to the treatment of CLBP. With the findings, we contribute to the development of an individualized approach to physiotherapy treatment and the optimization of physiotherapy approaches based on the psycho-physical characteristics of the vulnerable population of patients with CLBP. The study also includes an emphasis on trust as an important psychosocial factor in CLBP, which represents a new contribution to science.
