Title:Primerjava učinkovitosti vadbe mišic medeničnega dna in funkcionalne magnetne stimulacije pri zdravljenju stresne urinske inkontinence – pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Teoretična izhodišča: Stresna urinska inkontinenca (SUI) je najpogostejša oblika uhajanja urina, ki prizadene vsako tretjo žensko. Fizioterapija kot primarno metodo konservativnega zdravljenja uporablja vadbo mišic medeničnega dna (VMMD), v zadnjih desetih letih pa se vedno pogosteje uporablja tudi magnetna stimulacija. Namen diplomskega dela je bil primerjava učinkovitosti obeh metod pri zmanjšanju simptomov SUI ter analiza prednosti in slabosti obeh pristopov, kar bi lahko v prihodnje olajšalo odločitev za metodo zdravljenja. Metode: Empirični del temelji na kvalitativni raziskavi, pri čemer smo uporabili metodo kompilacije, deskripcije, komparacije in metodo analize ter sinteze. V raziskavo smo vključili 13 študij in en strokovni članek. Postopek iskanja izbranih študij smo predstavili s PRISMA diagramom. Kakovost izbranih člankov smo ocenili s pomočjo ocenjevalne lestvice PEDro. Glede na raziskovalna vprašanja smo analizirali raziskave in jih razdelili v tabelo kategorij in kod. Rezultati: Obe metodi sta se dokazali kot učinkoviti pri zmanjšanju simptomov SUI, krepitvi mišic medeničnega dna (MMD) in izboljšanju njihove vzdržljivosti, pri obeh se je spremenila morfološka lastnost sečil in pozitivni vpliv na izboljšanje dnevne kakovosti življenja. Primerjava obeh metod je pokazala večje razlike pri primerjavi prednosti in slabosti. Razprava: VMMD za učinkovito zdravljenje zahteva aktivno sodelovanje pacientke in pravilno izvedbo vaj, medtem ko se je MS izkazala za uporabno pri pacientih s slabim zaznavanjem MMD ali s trdovratnejšo obliko SUI. Po priporočilih EAU ostaja VMMD prva izbira zdravljenja, MS pa sekundarna metoda zaradi pomanjkanja kratko- in dolgoročnih raziskav o učinkovitosti.

Abstract:Theoretical Background: Stress urinary incontinence (SUI) is the most common form of urinary leakage, affecting one in three women. Physiotherapy, as the primary method of conservative treatment, traditionally employs pelvic floor muscle (PFM) training. However, in the past decade, magnetic stimulation (MS) has been increasingly used. The aim of this thesis was to compare the effectiveness of both methods in reducing SUI symptoms and to analyze the advantages and disadvantages of each approach, which may help inform treatment decisions in the future. Methods: The empirical part of the thesis is based on qualitative research, using compilation, description, comparison, and analysis and synthesis methods. The research included 13 studies and one expert article. The search process for selected studies was presented using a PRISMA diagram. The quality of the selected articles was assessed using the PEDro scale. Based on the research questions, the studies were analyzed and categorized in a table of categories and codes. Results: Both methods proved effective in reducing SUI symptoms, strengthening PFMs, and improving their endurance. Both also led to morphological changes in the urinary tract and had a positive impact on daily quality of life. The comparison of the two methods revealed more significant differences in terms of their respective advantages and disadvantages. Discussion: PFM training requires the patient's active participation and correct execution of exercises for effective treatment, whereas MS proved useful for patients with poor pelvic floor awareness or more persistent forms of SUI. According to EAU recommendations, PFM training remains the first-line treatment, while MS is considered a secondary method due to the lack of short- and long-term efficacy studies.
