Title:Fizioterapevtski pristop pri pacientu po transplantaciji srca – pregled literature in prikaz primera : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Teoretična izhodišča: Transplantacija srca je priznana kot učinkovit operativni poseg za bolnike z najtežjo obliko srčnega popuščanja, saj pomembno podaljša življenje prejemnika srčnega transplantata in izboljša njegovo kakovost življenja. Za uspešno okrevanje in povrnitev telesne zmogljivosti je potrebna celostna rehabilitacija, ki vključuje individualno prilagojeno fizioterapevtsko obravnavo. Namen: Na podlagi pregleda literature in prikaza primera zbrati in predstaviti podatke o najpogosteje uporabljenih fizioterapevtskih metodah pri obravnavi pacienta po transplantaciji srca in o omejitvah, ki se upoštevajo v obdobju fizioterapevtske obravnave pacienta po transplantaciji srca. Metode: Strokovno literaturo smo iskali v bazah PEDro in PubMed ter jo v našo raziskavo vključili na podlagi zastavljenih vključitvenih in izključitvenih kriterijev. Izvedli smo tudi študijo primera namenskega vzorca, ki je temeljila na terapijah in testiranjih. Rezultate izvedenih testov smo kvantitativno prikazali in kvalitativno interpretirali. V sklopu lastne raziskave smo izvedli tudi dva polstrukturirana intervjuja. Rezultati: Po pregledu literature smo v ožji izbor vključili štiri študije in rezultate pregledanih študij primerjali z lastno raziskavo. Rehabilitacija pacienta po transplantaciji srca vključuje različne metode in tehnike, ki jih fizioterapevt individualno prilagaja sposobnostim in omejitvam posameznega pacienta. To je ključnega pomena pri načrtovanju varne in uspešne fizioterapevtske obravnave pacienta po transplantaciji srca. Razprava: Ugotovili smo, da je visoko intenzivna intervalna vadba z nadzorom fizioterapevta varna in učinkovita metoda za izboljšanje funkcionalnosti in kakovosti življenja pacienta po transplantaciji srca, vendar so dolgoročni učinki sčasoma primerljivi z rezultati zmerno intenzivne kontinuirane vadbe. Celosten fizioterapevtski pristop vključuje nadzor fizioloških odzivov, postopno krepitev telesne zmogljivosti in spremljanje nezaželenih simptomov.

Abstract:Theoretical Background: Heart transplantation is an effective surgical procedure for patients with the most severe form of heart failure, as it significantly prolongs the recipient´s life and improves their quality of life. For a successful recovery and the restoration of physical capacity, a comprehensive rehabilitation is required, which includes an individually tailored physiotherapeutic approach. Purpose: To carry out a literature review and a case study and to collect and present data on the most commonly used physiotherapy methods in the rehabilitation of patients after heart transplantation. The aim was to research which limitations a physiotherapist must consider when working with a heart transplant recipient. Methods: We searched the PEDro and PubMed databases based on our inclusion and exclusion criteria for studies that were appropriate for our literature review. We also conducted a case study with a heart transplant recipient. We presented our numeric test results and interpreted them qualitatively. As part of our research, we also conducted two semi-structured interviews. Results: After the literature review, we chose four studies and compared the results of the reviewed studies with our own research. Rehabilitation of heart transplant patients includes various methods and techniques, which the physiotherapist individually adapts to the limitations of the patient. This is crucial in planning a safe and successful rehabilitation programme. Discussion: We found that high-intensity interval training under the supervision of a physiotherapist is a safe and effective method for improving functionality and quality of life in patients after heart transplantation. However, its long-term effects are eventually comparable to those of moderate-intensity continuous training. A comprehensive physiotherapy approach includes monitoring of physiological responses, gradual strengthening of physical capacity and observation of adverse symptoms.
