Abstract:Transplantacija srca je kompleksen kirurški poseg, ki je za paciente s končnim srčnim popuščanjem pogosto edina rešitev za podaljšano preživetje in izboljšanje kakovosti življenja. Ključno vlogo pri okrevanju pacientov imajo medicinske sestre, ki zagotavljajo celostno nego, vključno s fizično podporo, spremljanjem zdravilne terapije (kot so imunosupresivi) in obvladovanjem simptomov. Po presaditvi so pacienti izpostavljeni različnim izzivom, kot so tveganje zavrnitve presadka, okužbe in fizična šibkost, kar zahteva stalno medicinsko spremljanje in rehabilitacijo. Za uspešno okrevanje je potrebna celovita zdravstvena oskrba, ki vključuje izobraževanje o zdravem življenjskem slogu in preprečevanju zapletov. Poleg tega je pri skrbi za pacienta pomembna vključenost družine, saj nudi čustveno podporo in pomaga pri prilagajanju na življenje s presajenim srcem. Namen diplomske naloge je predstaviti vlogo medicinske sestre pri celostni obravnavi pacienta po transplantaciji srca, s poudarkom na okrevanju, preprečevanju zapletov ter pomenu izobraževanja in podpore pacientu in njegovi družini.

Title:MEDICAL CARE OF A PATIENT AFTER A HEARTH TRANSPLANT
Abstract:Heart transplantation is a complex surgical procedure that is often the only option for prolonged survival and improved quality of life in patients with end-stage heart failure. Nurses play a crucial role in the recovery process by providing comprehensive care, including physical support, monitoring of pharmacological therapy (such as immunosuppressants), and symptom management. After the procedure, patients face various challenges, such as the risk of graft rejection, infections, and physical weakness, which require continuous medical monitoring and rehabilitation. Successful recovery demands holistic healthcare, incorporating education on a healthy lifestyle and prevention of complications. Additionally, family involvement is essential in patient care, as it provides emotional support and helps the patient adapt to life with a transplanted heart. The purpose of this thesis is to present the nurse’s role in the comprehensive management of the patient after heart transplantation, with a focus on the recovery process, prevention of complications, and the importance of education and support for both the patient and their family.
