Title:Fizioterapevtska obravnava po operativni sanaciji rupture ahilove tetive – pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Teoretična izhodišča: Ahilova tetiva (AT) je največja in najdebelejša tetiva v človeškem telesu, vendar se kljub svoji velikosti pogosto poškoduje (Tarantino idr. 2020). Ruptura AT je definirana kot prekinitev kontinuitete, ki delno ali v celoti ogroža njeno strukturo in se lahko pojavi na katerikoli anatomski ravni, ki je med mišično-tetivnim stikom in narastiščem na tubesoritas calcaneusa, vendar se večina pojavi tam, kjer je oskrba s krvjo slaba, torej približno 2–6 cm nad pripenjališčem (Guevara-Chávez idr. 2024). V teoretičnem delu diplomskega dela predstavim anatomijo in strukturo Ahilove tetive, dejavnike tveganja za nastanek rupture Ahilove tetive, mehanizem nastanka in fizioterapevtske pristope po operativni sanaciji rupture Ahilove tetive. Metode: Raziskovalni del diplomskega dela je teoretična raziskava s kvalitativno metodologijo. Strokovno znanstveno literaturo sem iskala v podatkovnih bazah PubMed, ScienceDirect, PEDro, Google Scholar, ProQuest, ResearchGate in Scopus. Pri iskanju literature sem uporabila ključne besede: »Achilles tendon rupture«, »surgical treatment of Achilles tendon rupture« in »postoperative rehabilitation protocol of Achilles tendon rupture«. Pregled literature je trajal od leta 2015 do leta 2025. Rezultati: Raziskave smo izbrali z vključitvenimi in izključitvenimi kriteriji preko sistema PRISMA. V raziskavo smo vključili 16 raziskav s področja rehabilitacije po operativni sanaciji rupture Ahilove tetive. Razprava: Raziskave kažejo, da je pospešena funkcionalna rehabilitacija uspešna pri zmanjševanju postoperativnih zapletov, pripomore k uspešnim funkcionalnim izidom zdravljenja in k večjemu zadovoljstvu pacientov. Prav tako je bila izpostavljena uporaba tehnik, kot sta BFR (angl. blood flow restriction training) in nevro-mišična električna stimulacija za zmanjšanje atrofije mečne muskulature in ponovnega pridobivanja moči ter mišične mase.

Abstract:Background: The Achilles tendon (AT) is the largest and thickest tendon in the human body, but despite its size, it is frequently injured (Tarantino et al. 2020). An AT rupture is defined as a discontinuity that partially or completely compromises its structure and can occur at any anatomical level between the musculotendinous junction and the insertion on the calcaneal tuberosity. In the theoretical part of this thesis, I present the anatomy and structure of the Achilles tendon, the risk factors for Achilles tendon rupture, the mechanism of injury, and physiotherapeutic approaches following surgical repair of an Achilles tendon rupture. Methods: The research design of this thesis is a theoretical study using a qualitative methodology. I searched for scientific and professional literature in the databases PubMed, Science Direct, PEDro, Google Scholar, ProQuest, ResearchGate, and Scopus. In the literature search, I used the keywords: “Achilles tendon rupture,” “surgical treatment of Achilles tendon rupture,” and “postoperative rehabilitation protocol of Achilles tendon rupture.” The literature review covers the period from 2015 to 2025. Results: The studies were selected using inclusion and exclusion criteria through the PRISMA system. A total of 16 studies related to rehabilitation following surgical repair of Achilles tendon rupture were included in the research. Discussion: Studies show that accelerated functional rehabilitation is effective in reducing postoperative complications, contributes to successful functional treatment outcomes, and increases patient satisfaction. Additionally, the use of techniques such as BFR (blood flow restriction training) and neuromuscular electrical stimulation has been highlighted for reducing calf muscle atrophy and aiding in the recovery of strength and muscle mass.
