Title:Pomen hidroterapije pri totalni endoprotezi kolčnega sklepa

Abstract:Vstavitev totalne endoproteze kolčnega sklepa je pogost in uspešen ortopedski poseg, ki ima visoko stopnjo zadovoljstva med pacienti. Z naraščanjem števila degenerativnih bolezni pri starejši populaciji, aktivni življenjski slog in posledičnimi poškodbami pa narašča tudi število vstavljenih kolčnih endoprotez. V diplomskem delu smo predstavili totalno endoprotezo kolka kot operativni poseg in opisali funkcionalno stanje pacienta pred ter po njem. Glede na zdravnisˇko pooperativno oceno in dejavnike tveganja se pacienta vključi v program rehabilitacije, ki mu omogoča najbolj optimalen napredek v procesu okrevanja po vstavitvi TEP kolka. V diplomski nalogi smo tako predstavili pooperativno rehabilitacijo, ki je ključen vidik procesa okrevanja po operaciji TEP kolka. Hidroterapija se marsikje že uporablja kot oblika fizioterapevtske obravnave, vendar na tem področju še ni dovolj raziskav, ki bi zagotovile hidroterapiji mesto med najbolj uporabljenimi fizioterapevtskimi metodami pri rehabilitaciji pacienta. Raziskavo smo izvedli v Zdravilišču Laško, kjer smo izbrali 40 sodelujočih. Pri pacientih s totalno endoprotezo kolka smo preučevali pomen proste vadbe v vodi in uporabe podvodnih naprav kot oblike fizioterapevtske obravnave na obseg gibljivosti in stopnje bolečine v primerjavi z kinezioterapevtskimi metodami in tehnikami na suhem. Potrebnih je več visokokakovostnih študij z večjim vzorcem in daljšim časom spremljanja, da bi dosegli natančnejše in dolgoročnejše učinke. Prišli smo do ugotovitve, da je hidroterapija učinkovita pri povečanju obsega gibljivosti v kolčnem sklepu, zmanjšanju bolečine in izboljšanju funkcionalnosti v pooperativni fazi rehabilitacije pri bolnikih s totalno endoprotezo kolka. Vendar pa je zaradi številnih kontraindikacij ne morejo prejeti številni pacienti. Zdravilišče Laško je edino zdravilišče v Sloveniji, ki se že vrsto let poslužuje vadbe s podvodnimi napravami. Opravljena raziskava naj bo vzpodbuda tudi ostalim ustanovam o umestitvi te metode v rehabilitacijski proces.

Title:Effect of hydrotherapy on total arthroplasty of the hip joint
Abstract:Total hip arthroplasty is a common and successful orthopedic procedure that has a high level of patient satisfaction. With the increase in the number of degenerative diseases in the elderly population, the active lifestyle and the resulting injuries, the number of inserted hip endoprostheses is also increasing. In the thesis, we presented the total hip endoprosthesis as an operative intervention and described the functional state of the patient before and after it. Depending on the doctor's post-operative assessment and risk factors, the patient is included in a rehabilitation program that allows him to make the most optimal progress in the recovery process after TEP hip insertion. In this thesis, we presented postoperative rehabilitation, which is a key aspect of the recovery process after TEP hip surgery. Hydrotherapy is already used in many places as a form of physiotherapeutic treatment, but there is still not enough research in this field to guarantee hydrotherapy a place among the most used physiotherapeutic methods in patient rehabilitation. The research was conducted in the Laško health resort, where we selected 40 participants. In patients with total hip arthroplasty, we studied the importance of free exercise in water and the use of underwater devices as a form of physiotherapeutic treatment on range of motion and pain levels compared to kinesiotherapy methods and techniques on dry land. More high-quality studies with a larger sample size and longer follow-up time are needed to obtain more precise and longer-term effects. We came to the conclusion that hydrotherapy is effective in increasing the range of motion in the hip joint, reducing pain and improving functionality in the postoperative phase of rehabilitation in patients with total hip arthroplasty. However, many patients cannot receive it due to many contraindications. Zdravilišče Laško is the only spa in Slovenia that has been using underwater equipment for many years. The completed research should also encourage other institutions to incorporate this method into the rehabilitation process.
