Abstract:
Namen raziskave je bil preučiti učinkovitost TENS-a pri bolnikih s kronično bolečino v križu. V raziskavo smo vključili 80 preiskovancev. Rezultati so pokazali, da uporaba TENS-a osemkrat po 20 minut v obdobju 14 dni zmanjša stopnjo bolečine, merjene z vizualno analogno lestvico. Prav tako smo ugotovili, da uporaba TENS-a poveča funkcionalne zmožnosti, ocenjene po modificiranem Roland-Morrisovem vprašalniku. Na podlagi pridobljenih rezultatov sklepamo, da je pozitiven učinek terapije posledica naravnega poteka bolezni v času trajanja terapij. Kratkotrajen protibolečinski učinek s TENS-om smo po našem mnenju dosegli po teoriji kontrole vrat in zaradi porasta ravni endorfinov v cerebrospinalni tekočini, ki blažijo bolečino po naravni poti. Količina endorfinov se poveča v trenutkih zadovoljstva in delujejo tudi pri placebu, ko se ustvarja analgezija s samoaktivacijo endogene kontrole bolečine. Rezultate naše raziskave smo primerjali z drugimi raziskavami, ki smo jih pregledali, in ugotovili, da večina študij ni pokazala, da je TENS učinkovita terapija za lajšanje kronične bolečine v križu. Nekatere raziskave pa so vendarle dokazale učinkovitost TENS-a kot terapevtske metode. Sklepamo, da bi bilo treba opraviti več kakovostnih randomiziranih raziskav, ki bi bile dvojno slepe in bi imele kontrolo s placebom. Prav tako bi bilo treba izpeljati čim več ponovljivih raziskav, ki bi uporabile iste modifikacije TENS-a, čas trajanja terapije, vzorec preiskovancev, vključitvena in izključitvena merila, vprašalnike, saj bi s tem natančneje pokazale delovanje TENS-a kot terapevtske metode, z bolj izpopolnjenimi in pojasnjenimi rezultati.

Title:
The impact of transcutaneous electrical neuromuscular stimulation on chronic low back pain
Abstract:
The purpose of the study was to examine the efficacy of Transcutaneous Electrical Nerve Stimulation (TENS) for patients with chronic lower back pain. The study was conducted on 80 patients. The results show that the use of TENS, 8 times for 20 minutes over 14 days reduce the level of pain significantly, as measured by the Visual Analogue Scale. We also found that the use of TENS significantly increases the functional capabilities measured with the modified Roland Morris questionary. Based on the obtained results, we concluded that the positive effect of therapy was due to the natural course of the disease during the duration of the therapies. The short-term analgesic effect with TENS can be explained with the gate control theory and with increased levels of endorphin in the cerebrospinal fluid that reduced pain naturally. Endorphins increase in moments of pleasure, they can also be released because of a placebo, when analgesia is generated via self-activation of endogenous pain control. The results of our study were compared with other studies that we reviewed. We concluded that in most studies TENS was not found to be an effective therapy for the treatment of chronic lower back pain. However, some studies have demonstrated the effectiveness of TENS as a therapeutic method. In conclusion, more qualitative, randomised, double-blind, placebo-controlled trials are required. As much reproducible research should be carried out using the same modifications of TENS, duration of therapy, the sample of subjects, inclusion and exclusion criteria and questionnaires, thus giving a more concrete answer to the functioning of TENS as a therapeutic method with more sophisticated and explained results.
