Title:Vpliv fizioterapevtske obravnave pri zdravljenju urogenitalne atrofije

Abstract:Urogenitalna atrofija je le ena izmed mnogih sprememb, ki doletijo ženske v času pomenopavze. Pojavi se ob prvih znakih pomanjkanja estrogena. Urogenitalne spremembe ob nastopu prizadenejo zunanje spolovilo, nožnico in vse mišične strukture medeničnega dna. Občutljivost, srbečica, pekoč občutek, suhost, disparevnija, vse to so značilni znaki in simptomi urogenitalne atrofije, ki v času menopavze doleti kar 60 odstotkov žensk in vpliva na kakovost njihovega življenja. Pomembno je pomoč poiskati že ob prvem pojavu simptomov. V zgodnji fazi zdravljenje v večini primerov poteka konzervativno, in sicer z vajami za krepitev mišic medeničnega dna, ki se lahko izvajajo s pomočjo pripomočkov ali brez njih. Če se ta oblika zdravljenja ne izkaže za dovolj učinkovito, poizkusijo z zdravljenjem z estrogeni. V diplomskem delu smo predstavili optimalen trening mišic medeničnega dna, pridruženega z različnimi fizioterapevtskimi metodami in tehnikami, ki lahko pripomorejo k boljšemu zavedanju in delovanju mišic medeničnega dna in tako tudi k izboljšanju simptomov UA. Za učenje pravilne kontrakcije mišic se lahko poslužujemo uporabe različnih metod in tehnik. Ena izmed teh je tudi biološka povratna zveza (biofeedback), ki jo lahko uporabimo kot dodatek k vadbi, pod nadzorom fizioterapevta. Pri izvajanju terapije je na začetku izrednega pomena vloga fizioterapevta, da pacienta nauči pravilne aktivacije mišic ter določi smernice fizioterapevtske obravnave, ki jih lahko med obravnavo prilagaja glede na pacientove zmožnosti in cilje.

Title:The role of physiotherapeutic treatment of urogenital atrophy
Abstract:Urogenital atrophy is just one of the many changes that affect women during post-menopause. The first signs of estrogen deficiency appear. Urogenital changes affect the genitalia, vagina and all the muscular structures of the pelvic floor. Tenderness, itching, burning, dryness, dyspareunia are all typical signs and symptoms of urogenital atrophy, which affects 60 percentage of women during menopause and affects their quality of life. It is important to seek help at the first sign of symptoms. In the early stages, treatment in most cases is conservative, with exercises to strengthen the pelvic floor muscles, which can be done with or without the use of devices. If this form of treatment does not prove effective enough, oestrogen therapy is tried. In this thesis we have presented optimal training of the pelvic floor muscles associated with different physiotherapeutic methods and techniques that can help to improve the awareness and functioning of the pelvic floor muscles. Different methods and techniques can be used to teach the correct contraction of the muscles. One of these is biofeedback, which can be used as an adjunct to exercise, under the supervision of a physiotherapist. The role of the physiotherapist is of paramount importance in the initial stages of the therapy, in order to teach the patient the correct way to activate the muscles and to establish guidelines for the physiotherapy treatment, which can be adapted during the course of the treatment according to the patient's abilities and goals.
