Title:UČINKOVITOST KOMBINACIJE IZOBRAŽEVANJA O NEVROZNANOSTI BOLEČINE IN TERAPEVTSKIH VAJ PRI PACIENTIH S KRONIČNO MIŠIČNO-SKELETNO BOLEČINO

Abstract:Na primarni zdravstveni ravni je KMSB eden najpogostejših razlogov za posvet pri zdravniku. Terapevtski posegi, ki temeljijo na aktivnih strategijah obvladovanja, so bistveni za zdravljenje kronične bolečine in vzdržnost javnega zdravstvenega sistema. Večina terapevtskih metod za obravnavo kronične bolečine je farmakoloških in invazivnih, kar pa ni v skladu s smernicami, ki v prvi vrsti priporočajo obravnavo kronične bolečine z uporabo edukacije in izvajanjem terapevtskih vaj. Pristop k obravnavi pacienta s kronično bolečino mora biti celosten, kar spada med načela biopsihosocialnega modela. Biopsihosocialni model upošteva biološke, psihološke in socialne dejavnike, ki vplivajo na dojemanje bolečine. Izobraževanje o nevroznanosti bolečine je metoda, s katero paciente naučimo razumevati nevrofiziologijo bolečine z namenom, da bi znanje uporabili kot strategijo za obvladovanje simptomov pri spoprijemanju s kronično bolečino. Mehanizem delovanja INB je rekonceptualizacija bolečine. Učinkovitost uporabe zgolj INB ni pokazala statistično značilnih izboljšav v intenzivnosti bolečine, stopnji invalidnosti ali psihosocialnih spremenljivk. Obstajajo zanesljivi znanstveni dokazi o učinkovitosti INB, kadar je v kombinaciji s terapevtskimi vajami ali drugimi manualnimi terapijami. Izobraževanje o nevroznanosti bolečine v kombinaciji s terapevtskimi vajami običajno prinaša boljše rezultate kot zgolj terapevtske vaje. Priporočena je uporaba standardnih fizioterapevtskih postopkov v kombinaciji z INB, pri čemer INB učinkovito odpravlja psihosocialne omejitve, terapevtska vadba pa telesne omejitve. V do sedaj objavljenih raziskavah ni enotnega načina izvajanja INB. Da bi lahko podali zanesljiva priporočila za uporabo v kliničnem okolju, se priporoča nadaljnjo raziskovanje področja.

Title:The effectiveness of combined neuroscience education and therapeutic exercises in patients with chronic musculoskeletal pain
Abstract:At the primary health care level, chronic muscoskeletal pain is one of the most common reasons for a doctor's consultation. Therapeutic interventions based on active coping strategies are essential for the treatment of chronic pain and the sustainability of the public health system. Most of the therapeutic methods for dealing with chronic pain are pharmacological and invasive, which is not in accordance with the guidelines, which primarily recommend dealing with chronic pain through the use of education and the implementation of therapeutic exercises. The approach to treating a patient with chronic pain must be holistic, which is one of the principles of the biopsychosocial model. The biopsychosocial model takes into account biological, psychological and social factors that influence the perception of pain. Pain neuroscience education is a method of teaching patients to understand the neurophysiology of pain in order to use the knowledge as a symptom management strategy for coping with chronic pain. The mechanism of action of PNE is the reconceptualization of pain. The efficacy of using PNE alone did not show statistically significant improvements in pain intensity, disability level, or psychosocial variables. There is solid scientific evidence for the effectiveness of PNE when combined with therapeutic exercises or other manual therapies. Education about the neuroscience of pain combined with therapeutic exercises usually produces better results than therapeutic exercises alone. It is recommended to use standard physiotherapy procedures in combination with PNE, whereby PNE effectively eliminates psychosocial limitations and therapeutic exercise physical limitations. In the research published so far, there is no uniform way of performing PNE. In order to be able to make reliable recommendations for use in the clinical setting, further research in the field is recommended.
