Title:VLOGA MEDICINSKE SESTRE PRI POOPERATIVNEM NADZORU PACIENTA

Abstract:Teoretična izhodišča: Pooperativno obdobje je pomembna faza v procesu okrevanja pacienta po operaciji. Medicinske sestre so odgovorne za spremljanje vitalnih znakov, obvladovanje bolečine, opazovanje operativnega mesta ter zaznavanje zapletov, kar je ključno za uspešno okrevanje pacienta. Pooperativni nadzor vključuje natančne postopke in intervencije, katerih cilj je preprečiti morebitne zaplete in zagotoviti optimalno zdravljenje. Kontinuirano strokovno izpopolnjevanje je pomembno za učinkovito izvajanje teh nalog in prispeva k izboljšanju kakovosti oskrbe ter zadovoljstvu pacienta. Namen diplomske naloge je preučiti vlogo medicinske sestre pri pooperativnem nadzoru pacienta. Metode: Raziskava je temeljila na kvalitativnem raziskovalnem pristopu in deskriptivni metodi dela. Kot inštrument za zbiranje podatkov smo izdelali predlogo za polstrukturirani intervju. Primarni podatki so bili pridobljeni s tehniko intervjuvanja. Vzorec je bil namenski in je vključeval diplomirane medicinske sestre z vsaj 5 leti delovnih izkušenj v pooperativnem nadzoru pacienta. V intervju smo zajeli šest diplomiranih medicinskih sester. Raziskava je potekala v juliju 2024. Rezultati: Za vse intervjuvanke pooperativni nadzor obsega spremljanje vitalnih funkcij, ocenjevanje bolečine ter opazovanje operativne rane oz. operativnega mesta, vse to pa predstavlja tudi ključne naloge za večino intervjuvank. Vitalne funkcije vse intervjuvanke spremljajo s pomočjo monitoringa pacienta na 30 min. Najpogostejši diagnostično-terapevtski postopki za večino intervjuvank so aplikacija intravenozne terapije, opazovanje operativnega mesta in obvladovanje bolečine. Večini intervjuvank uspešen pooperativni nadzor predstavljajo stabilne vitalne funkcije, obvladovana bolečina in dobro splošno počutje pacienta. Pooperativne zaplete intervjuvanke prepoznajo po spremembah v vitalnih funkcijah, bolečini, videzu operativnega mesta in po počutju pacienta, ob opaženih spremembah o tem obvestijo anesteziologa ali operaterja in delujejo po njegovih navodilih. Vse intervjuvanke se udeležujejo izobraževanj, ki jim pomagajo pri bolj kakovostnem pooperativnem nadzoru. Razprava: V raziskavi smo preučili vlogo medicinske sestre pri pooperativnem nadzoru pacienta, podprto s strokovno literaturo s tega področja. Raziskava se je osredotočila na naloge medicinskih sester, postopke pooperativnega nadzora, obravnavo zapletov po operaciji in pomen kontinuiranega strokovnega izpopolnjevanja. Ključne naloge vključujejo spremljanje vitalnih funkcij, ocenjevanje bolečine, opazovanje operativne rane ter skrb za splošno udobje pacienta. Ugotovili smo, da so v postopkih pooperativnega nadzora najpomembnejše aktivnosti, kot so spremljanje vitalnih znakov, obvladovanje bolečine ter preprečevanje zapletov. Strokovno izpopolnjevanje izboljšuje kompetence medicinskih sester, kar vodi k boljši kakovosti oskrbe in večji sposobnosti prepoznavanja zapletov. Naša raziskava je tudi pokazala, da so redna izobraževanja ključna za učinkovito izvajanje pooperativnega nadzora ter za izboljšanje oskrbe in okrevanja za paciente.

Title:THE ROLE OF THE NURSE IN THE POSTOPERATIVE CONTROL OF THE PATIENT
Abstract:Background: The postoperative period is a critical phase in the patient’s recovery process following surgery. Nurses are responsible for monitoring vital signs, managing pain, observing the surgical site, and detecting complications, all of which are crucial for the patient’s successful recovery. Postoperative monitoring involves precise procedures and interventions aimed at preventing potential complications and ensuring optimal care. Continuous professional development is essential for effectively performing these tasks and contributes to enhancing the quality of care and patient satisfaction. The purpose of this thesis is to examine the role of the nurse in postoperative patient monitoring. Methods: The research was based on a qualitative research approach and a descriptive method. We used a template for a semi-structured interview as the data collection instrument. Primary data were obtained through interviewing. The sample was purposive and included registered nurses with at least 5 years of experience in postoperative patient monitoring. Six registered nurses were interviewed. The research was conducted in July 2024. Results: For all the interviewees, postoperative monitoring includes tracking vital signs, assessing pain, and observing the surgical site or operative area, which also constitutes the key tasks for most of the interviewees. All the interviewees monitor vital signs through patient monitoring every 30 minutes. The most common diagnostic and therapeutic procedures for most of the interviewees are the administration of intravenous therapy, observing the surgical site, and pain management. For most interviewees, successful postoperative monitoring is characterized by stable vital signs, controlled pain, and the patient's overall well-being. Postoperative complications are recognized by changes in vital signs, pain, the appearance of the surgical site, and the patient's condition. Upon noticing these changes, the interviewees notify the anesthesiologist or surgeon and act according to their instructions. All interviewees participate in educational programs that assist them in providing higher-quality postoperative monitoring. Discussion: In this study, we examined the role of the nurse in postoperative patient monitoring, supported by professional literature in the field. The research focused on the responsibilities of nurses, postoperative monitoring procedures, management of postoperative complications, and the importance of continuous professional development. Key tasks include monitoring vital signs, assessing pain, observing the surgical site, and ensuring the patient’s overall comfort. We found that the most important activities in postoperative monitoring are vital signs assessment, pain management, and complication prevention. Professional development enhances nurses' competencies, leading to improved care quality and better ability to recognize complications. Our study also highlighted that regular education is crucial for effective postoperative monitoring and for enhancing patient care and recovery.
