Title:ZDRAVSTVENA NEGA BOLNIKA Z REZISTENTNO BAKTERIJO V BOLNIŠNIČNEM OKOLJU

Abstract:Izhodišča: Rezistentne bakterije predstavljajo resno grožnjo v sodobnem zdravstvu, saj se učinkovito upirajo zdravljenju z antibiotiki. S celovitim pristopom k zdravstveni negi bolnikov z rezistentnimi bakterijami je mogoče doseči izboljšanje kvalitete oskrbe, zmanjšanje širjenja okužb in ohranjanje učinkovitosti zdravljenja v prihodnosti. Namen raziskave je bil raziskati zdravstveno nego bolnika z rezistentno bakterijo v bolnišničnem okolju, cilji pa so bili raziskati aktivnosti in izzive bolnikov v zdravstveni negi ter ugotoviti posebnosti, s katerimi se srečujejo zdravstveni delavci pri obravnavi bolnika z rezistentno bakterijo v bolnišničnem okolju. Metode: Raziskava je temeljila na kvalitativnem pristopu raziskovanja in deskriptivni metodi dela. Za potrebe teoretičnega dela so bili zbrani primarni in sekundarni viri, pridobljeni iz podatkovnih baz (Cobiss, CINAHL, PubMed, ResearchGate, Google Učenjak). Podatke za raziskavo smo zbrali s pomočjo polstrukturiranega intervjuja, ki je vključeval 6 medicinskih sester z različnih bolnišničnih oddelkov, starejših od 25 let in z vsaj dvema letoma delovnih izkušenj v kliničnem okolju. Rezultati: Anketiranci navajajo, da so najpomebnejše aktivnosti zdravstvene nege individualizirani načrti zdravstvene nege glede na vrsto in mesto okužbe z rezistentno bakterijo, dosledno upoštevanje priporočil stroke, skrb za nego rane koloniziranega bolnika, izobraževanje zaposlenih, pacientov in njihovih družin, kakovostno čiščenje in razkuževanje, omejena izbira antibiotikov ter preprečevanje njihove prekomerne rabe in zmanjševanje strahu pred okužbo izvajalcev zdravstvene nege. Razprava: Ugotovili smo, da je pri izvajanju zdravstvene nege bolnika z rezistentno bakterijo v bolnišničnem okolju ključna uporaba osebne varovalne opreme, prav tako pa dosledno izvajanje higiene rok, individualni pristop, personaliziran načrt zdravstvene nege, izvedba ustrezne osamitve, ločevanje čistih in nečistih poti ter neprestano izobraževanje izvajalcev zdravstvene nege. Vse to je bistveno za doseganje najboljših rezultatov v smislu zdravljenja in preprečevanja širjenja bolnišničnih okužb.

Title:MEDICAL CARE OF A PATIENT WITH RESISTANT BACTERIA IN A HOSPITAL ENVIRONMENT
Abstract:Background: Resistant bacteria pose a serious threat in modern healthcare, as they effectively resist antibiotic treatment. With a comprehensive approach to the health care of patients with resistant bacteria, it is possible to improve the quality of care, reduce the spread of infections and maintain the effectiveness of treatment in the future. The purpose of the research was to investigate the health care of a patient with resistant bacteria in a hospital environment, and the goals were to explore the activities and challenges of patients in nursing care and to determine the specifics that healthcare professionals encounter when treating a patient with resistant bacteria in a hospital environment. Methods: The research was based on a qualitative research approach and a descriptive work method. For the theoretical part, primary and secondary sources were collected from databases (Cobiss, CINAHL, PubMed, ResearchGate, Google Scholar). Data for the research were gathered through semi-structured interviews, which involved 6 nurses from different hospital departments, all over the age of 25 and with at least two years of clinical work experience. Results: The respondents stated that the most important nursing activities are individualized care plans based on the type and location of the resistant bacterial infection, consistent adherence to professional guidelines, care of wounds in colonized patients, education of staff, patients, and their families, quality cleaning and disinfection, limited selection and overuse of antibiotics, and, most importantly, the fear of infection among healthcare providers. Discussion: : We found that the use of personal protective equipment, consistent hand hygiene, an individualized approach, a personalized care plan, the implementation of appropriate isolation and the separation of clean and unclean pathways, and continuous education of health care providers are essential to achieve the best results in terms of treatment and prevention of the spread of nosocomial infections in the hospital environment.
