Title:Upoštevanje načel k pacientu usmerjene oskrbe v kliničnem okolju

Abstract:Izhodišča: Raziskava se osredotoča na preučevanje uresničevanja koncepta k pacientu usmerjene oskrbe v kliničnem okolju, pri čemer se postavlja vprašanje, kako se ta koncept, ki poudarja individualne potrebe in želje pacienta, dejansko implementira v vsakodnevni praksi zdravstvenih izvajalcev. Namen raziskave je zagotoviti celovito analizo trenutnega stanja k pacientu usmerjene oskrbe v kliničnem okolju ter raziskati, v kolikšni meri so načela k pacientu usmerjene oskrbe, ki jih je oblikoval Picker, integrirana v obravnavo pacientov. Cilji raziskave vključujejo preučitev znanstvene in strokovne literature, ki se osredotoča na koncept k pacientu usmerjene oskrbe, ugotavljanje, kako se Pickerjeva načela upoštevajo v praksi, ter identificiranje, katera načela so v kliničnem okolju upoštevana in katera ne. Poleg tega nameravamo raziskati vzroke za morebitno neupoštevanje teh načel, kar bi lahko razkrilo ovire in izzive, s katerimi se zdravstveni izvajalci srečujejo pri izvajanju k pacientu usmerjene oskrbe. S temi raziskovalnimi cilji želimo prispevati k razumevanju in izboljšanju k pacientu usmerjene oskrbe v praksi ter k razvoju strategij za boljšo implementacijo tega pristopa v zdravstvenih ustanovah. Metode: V raziskavi smo uporabili kvalitativno metodo zbiranja podatkov s poglobljenimi intervjuji z dvema zdravnikoma, desetimi diplomiranimi medicinskimi sestrami in tremi zdravstvenimi tehniki, zaposlenimi v terciarni zdravstveni ustanovi na različnih internističnih in kirurških oddelkih ter na urgentnem internističnem oddelku. Intervjuji so potekali v vnaprej dogovorjenem prostoru, ki so ga izbrali zdravstveni strokovnjaki sami. Intervjuje smo z dovoljenjem intervjuvancev snemali in jih nato prepisali ter anonimizirali (I/1 do I/15). Prepisi intervjujev so bili večkrat prebrani, nato so sledili kodiranje, izbor in opredelitev pojmov ter postopek definiranja podteme in teme. Rezultati: Večina intervjuvancev razume k pacientu usmerjeno oskrbo kot pozitiven pristop v zdravstveni obravnavi pacienta, vendar intervjuvanci izpostavljajo ovire, ki jim preprečujejo izvajanje k pacientu usmerjene oskrbe. Kot najpogostejše so izpostavili: pomanjkanje časa, premalo osebja, stres na delovnem mestu, pomanjkanje znanja o k pacientu usmerjeni obravnavi, slabo komunikacijo, izgorelost in utrujenost, kulturo oddelka ali organizacije, nezainteresiranost za spremembe. Razprava: Glede na pridobljene podatke bi bilo treba zdravstvenim strokovnjakom zagotoviti dodatno izobraževanje, s katerim bi jih opolnomočili za izvajanje k pacientu usmerjene oskrbe. Raziskava tudi poudarja pomen komunikacije in sodelovanja zdravstvenih delavcev v oskrbi, zlasti pri starejših osebah, kjer sta zaupanje in spoštovanje ključnega pomena. Komunikacija, ki temelji na zaupanju, vpliva na izboljšanje splošnega počutja pacientov, hitrost okrevanja, upoštevanje zdravstvenih navodil ter boljšo kakovost življenja.

Title:Adherence to the principles of patient-centered care in the clinical setting
Abstract:Background: The research focuses on the implementation of the concept of patient-centred care in the clinical setting, asking how this concept, which emphasises the individual needs and preferences of the patient, is actually implemented in the daily practice of healthcare providers. The aim of the study is to provide a comprehensive analysis of the current state of patient-centred care in the clinical setting and to investigate to what extent the principles of patient-centred care, as formulated by Picker, are integrated into the management of patients. The objectives of the research include examining the scientific and professional literature focusing on the concept of patient-centred care, identifying how Picker's principles are followed in practice, and identifying which principles are and are not followed in the clinical setting. In addition, we aim to explore the reasons for possible non-adherence to these principles, which could reveal the barriers and challenges that healthcare providers face in implementing patient-centred care. Through these research objectives, we aim to contribute to understanding and improving patient-centred care in practice and to developing strategies for better implementation of this approach in healthcare settings. Methods: The study used a qualitative method to collect data through in-depth interviews with two doctors, ten registered nurses and three health technicians working in a tertiary care facility in different internal medicine and surgery departments, as well as in an emergency internal medicine department. The interviews took place in a pre-arranged venue chosen by the health professionals themselves. With the interviewees' permission, the interviews were recorded and then transcribed and anonymised (I/1 to I/15). The interview transcripts were read several times, followed by coding, selection and definition of terms, and a process of defining the subtheme and theme. Results: The majority of interviewees perceived patient-centred care as a positive approach in patient care, but highlighted barriers that prevent interviewees from implementing patient-centred care. The most common barriers were: lack of time, too few staff, stress in the workplace, lack of knowledge about patient-centred care, poor communication, burnout and fatigue, culture of the department or organisation, lack of interest in change. Discussion: In the light of the findings, additional training should be provided to empower healthcare professionals to deliver patient-centred care. The study also highlights the importance of communication and collaboration among healthcare professionals in care, especially for older people, where trust and respect are key. Communication based on trust has an impact on improving patients' overall well-being, speed of recovery, adherence to medical instructions and quality of life.
