Title:Fizioterapevtska obravnava delno strgane tetive supraspinatusa : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Teoretična izhodišča: Delna raztrganina tetive supraspinatusa velja za eno izmed najpogostejših poškodb ramenskega obroča. Incidenca poškodbe narašča s starostjo. Najpogosteje je vzrok degeneracija tkiva, preobremenitev ali akutna travma. Smernice rehabilitacije priporočajo konservativni rehabilitacijski pristop, ki zajema tudi fizioterapevtsko obravnavo. V začetni fazi je priporočljiv počitek in prilagoditev aktivnosti, nato pa terapija z različnimi manualnimi tehnikami in fizikalnimi agensi. Namen: Namen diplomskega dela je bil na podlagi pregleda literature predstaviti učinkovito rehabilitacijo za delno strgano tetivo supraspinatusa. Metode dela: V empiričnem delu smo se poslužili kvalitativne raziskovalne metode. Ustrezno literaturo smo iskali v naslednjih podatkovnih bazah: PubMed, Science Direct, Google Scholar, Emerald Insight, ProQuest in PEDro. Za iskanje zadetkov smo zasnovali iskalni niz: Partial-thickness tear AND supraspinatus AND physiotherapy OR nonoperative treatment OR conservative rehabilitation OR physical therapy. V analizo smo vključili članke s prostim dostopom do celotnega besedila, objavljene leta 2014 ali kasneje in napisane v nam razumljivem jeziku. Rezultati: V končno analizo smo vključili 8 raziskav, ki so preiskovale učinkovitost in primernost različnih fizioterapevtskih metod in tehnik. V vseh raziskavah je bilo ugotovljeno, da je fizioterapevtska obravnava ustrezna izbira rehabilitacije delno strgane tetive supraspinatusa. Razprava: Za obravnavo delne raztrganine tetive supraspinatusa se najpogosteje uporablja terapevtski vadbeni program, terapija z udarnimi valovi, terapevtski ultrazvok, laser in TENS v kombinaciji s počitkom, prilagojeno aktivnostjo in medikamentozno terapijo. V prihodnje bi bilo smiselno izvesti raziskave na področju učinkovitosti manualnih tehnik pri dotični diagnozi.

Abstract:Theoretical background: A partial-thickness tear of the supraspinatus tendon is one of the most common injuries of the shoulder girdle. The incidence of this injury increases with age, with the main causes being tissue degeneration, overuse, or acute trauma. Rehabilitation guidelines recommend a conservative approach, which also includes physiotherapy. In the initial phase, rest and activity modification are recommended, followed by therapy using various manual techniques and physical agents. Purpose: The aim of this thesis was to present an effective rehabilitation approach for partial-thickness supraspinatus tendon tear based on a literature review. Work Methods: In the empirical part of thesis, we used a qualitative research method. Relevant literature was searched in the following databases: PubMed, Science Direct, Google Scholar, Emerald Insight, ProQuest, and PEDro. The search was based on the following string: Partial-thickness tear AND supraspinatus AND physiotherapy OR nonoperative treatment OR conservative rehabilitation OR physical therapy. Articles with free access to whole text, published in 2014 or later and written in a language we understand, were included in analysis. Results: The final analysis included 8 studies investigating the effectiveness and appropriateness of various physiotherapy methods and techniques. All studies confirmed that physiotherapy is an appropriate choice for rehabilitation of partial-thickness supraspinatus tendon tear. Discussion: The most used interventions for partial-thickness supraspinatus tendon tears include therapeutic exercise programs, shockwave therapy, therapeutic ultrasound, laser, and TENS, combined with rest, modified activity, and pharmacological therapy. Future research should investigate the effectiveness of manual techniques for this diagnosis.
