Title:SLAP lezija in fizioterapevtska obravnava : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Uvod: SLAP lezija (superiorni labrum, anterior do posterior) je poškodba proksimalnega narastišča dolge tetive bicepsa brachii skupaj z labrumom na superiornem glenoidu, do katere najpogosteje pride zaradi ponavljajočih se zamahov/metov nad glavo. Najpogosteje prizadene športnike, je pa lahko tudi posledica delovne preobremenitve, travmatičnega dogodka ali degenerativnih sprememb in se lahko pojavi pri ljudeh vseh starosti. Namen: Namen tega diplomskega dela je raziskati predvsem tujo strokovno literaturo, saj v slovenskem znanstvenem prostoru primanjkuje primernih študij, ki lahko pomagajo fizioterapevtom pri oblikovanju učinkovitega fizioterapevtskega protokola za zmanjševanje potrebe po operativnem zdravljenju SLAP lezije. Metode dela: Raziskovalna metodologija temelji na pregledu znanstvene in strokovne literature. Iskanje predvsem tuje literature je potekalo v podatkovnih zbirkah PubMed, Google Scholar, Science Direkt, ReasearchGate, SageJournals in Elsevier. Zanimale so me raziskave, napisane med letoma 2014 in 2024. Rezultati: S pomočjo diagrama PRIZMA sem izmed 1566 zadetkov izbrala 10 člankov, ki sem jih vključila v končni pregled. Pri iskanju sem uporabila ključne besede: “SLAP lesion”, “non-operative treatment”, “psysiotherapy treatment”, “shoulder injuries”. Razprava in zaključek: SLAP lezija se najprej zdravi konservativno od 3 do 6 mesecev. Šele če je konservativno zdravljenje neuspešno, se poslužimo operativnega zdravljenja. Avtorji izbranih člankov so v svojih raziskavah prišli do dokaj enotnih ugotovitev. Konservativna fizioterapija SLAP lezije sestoji iz raztezanja posteriorne sklepne kapsule, vaj za moč periskapularnih mišic, mišic rotatorne manšete, trupa in nog ter vaj za stabilizacijo skapule. Raziskave kažejo, da je v večini primerov konservativno zdravljenje zadostno, vendar je njegova učinkovitost odvisna od trajanja simptomov, poklica/športa, starosti in morebitnih pridruženih patologij.

Abstract:Introduction: SLAP lesion is an injury of the proximal process of the long biceps brachii tendon together with the labrum of the superior glenoid, most commonly caused by repetitive overhead swings/throws. It most commonly affects athletes, but can also result from work overload, traumatic event or degenerative changes and can occur to people of any age. Aim: The aim of this thesis is to investigate mainly foreign literature, as there is a lack of adequate studies in the Slovenian scientific area that can help physiotherapists in designing an effective physiotherapy protocol to prevent operative treatment of SLAP lesions. Methods: The research methodology is based on a review of the scientific literature, written between 2014 and 2024. The literature search was conducted mainly in PubMed, Google Scholar, Science Direct, ReasearchGate, SageJournals and Elsevier. Results: Using the PRIZMA diagram, 10 articles were selected from the total of 1566 hits to include in the final review. The search was conducted by using the following keywords: "SLAP lesion", "non-operative treatment", "physiotherapy treatment", "shoulder injuries". Discussion and conclusion: SLAP lesion is initially treated conservatively for 3 to 6 months. If conservative treatment is shown to be unsuccessful than many operative treatments are available. The authors of the selected articles have reached fairly uniform conclusions in their studies. Conservative physiotherapy for SLAP lesions consists of stretching of the posterior joint capsule, strength exercises for the periscapular muscles, rotator cuff muscles, trunk and leg muscles, and scapula stabilization exercises. Research shows that conservative treatment is sufficient in most cases, but its effectiveness depends on the duration of symptoms, occupation/sport, age and other possible associated pathologies.
