Title:UČINKOVITOST DELA PRVIH POSREDOVALCEV V SISTEMU NUJNE MEDICINSKE POMOČI

Abstract:Teoretična izhodišča: Prvi posredovalci so pomemben del sistema nujne medicinske pomoči v Sloveniji, še posebej v krajih, ki so od najbližje enote nujne medicinske pomoči bolj oddaljeni. Pri bolnikih, ki so življenjsko ogroženi, je hitra pomoč ključna za preživetje. Namen raziskave je analizirati delo prvih posredovalcev. Cilj raziskave je raziskati učinkovitost dela prvih posredovalcev v sistemu nujne medicinske pomoči. Metode: Za raziskavo smo uporabili kvantitativno metodo raziskovanja. Primarne podatke smo pridobili z analizo standardiziranih protokolov nujne medicinske pomoči Zdravstvenega doma Postojna. Analizirali smo naslednje protokole: Sprejem nujnih intervencij, Protokol nujne intervencije in Protokol predbolnišničnega oživljanja. Uporabili smo vse protokole, ki so bili izpolnjeni v obdobju od 1. 1. 2021 do 31. 12. 2024. Rezultati: Z raziskavo smo ugotovili, da so na območju Zdravstvenega doma Postojna v obdobju od 1. 1. 2021 do 31. 12. 2024 med vsemi intervencijami enote nujne medicinske pomoči (n = 5207) prvi posredovalci posredovali v 116 primerih. Največkrat so prvi posredovalci posredovali zaradi bolezenskih stanj (78 %), in sicer v 41 % zaradi nezavesti bolnika in odpovedi dihanja (36 %). V 35 % je bilo oživljanje bolnika tudi zaradi posredovanja prvih posredovalcev uspešno. V 96 % intervencij so prvi posredovalci prispeli na kraj dogodka pred ekipo nujne medicinske pomoči. Povprečen dostopni čas ekipe nujne medicinske pomoči Postojna na kraj je znašal 13 minut, medtem ko se dostopni čas prvih posredovalcev ne beleži. Razprava: Organiziran sistem prvih posredovalcev je ključ do skrajšanja dostopnih časov organizirane oblike pomoči bolnikom, ki lahko nudijo hitro in učinkovito pomoč v nujnih, življenjsko ogrožajočih stanjih. Tak sistem je ključen za preživetje in kasneje kakovostno življenje po odpustu iz bolnišnice. Prvi posredovalci so tisti, ki so lahko že nekaj minut po nastanku dogodka na kraju in nudijo neodložljive ukrepe do prihoda ekip nujne medicinske pomoči. Za delovanje sistema prvih posredovalcev je potreben majhen finančni vložek, ki daje dobre rezultate pri reševanju življenj.

Title:EFFECTIVENESS OF WORK OF FIRST RESPONDERS IN THE MERGENCY MEDICAL ASSISTANCE SYSTEM
Abstract:Theoretical Background: First responders are an important part of the emergency medical services system in Slovenia, especially in areas that are more distant from the nearest emer-gency medical unit. For patients in life-threatening situations, rapid assistance is crucial for survival. The purpose of this study is to analyze the work of first responders. The aim of the research is to investigate the effectiveness of first responders within the emergency medical services system. Methods: A quantitative research method was used for this study. Primary data were obtai-ned by analyzing standardized emergency medical service protocols from the Postojna Health Center. The following protocols were analyzed: Emergency Intervention Admission, Emer-gency Intervention Protocol, and Prehospital Resuscitation Protocol. All protocols completed between January 1, 2021, and December 31, 2024, were included. Results: The research found that, in the area served by the Postojna Health Center, first re-sponders intervened in 116 cases out of a total of 5,207 emergency medical unit interventions during the period from January 1, 2021, to December 31, 2024. Most often, first responders intervened due to medical conditions (78%), specifically in 41% of cases due to patient un-consciousness and in 36% due to respiratory arrest. In 35% of cases, resuscitation was suc-cessful as a result of the intervention by first responders. In 96% of the interventions, first responders arrived at the scene before the emergency medical team. The average response time of the Postojna emergency medical team was 13 minutes, while the response time of first re-sponders is not recorded. Discussion: An organized first responder system is key to reducing the response time of struc-tured assistance to patients, enabling the provision of rapid and effective help in emergency, life-threatening situations. Such a system is crucial for survival and for a good quality of life after hospital discharge. First responders can be on the scene within minutes of an incident and can provide essential immediate measures until the arrival of emergency medical teams. The operation of the first responder system requires minimal financial investment but delivers strong results in saving lives.
