Title:Podaljšano statično raztezanje v procesu rehabilitacije : pregled literature

Abstract:Podaljšano statično raztezanje (PSR) je ena izmed tehnik za izboljšanje gibljivosti, vendar pa sta njena učinkovitost v rehabilitaciji in vplivi na poškodovano tkivo manj (po)znani. Namen te diplomske naloge je bil s sistematičnim pregledom literature ugotoviti vpliv PSR na poškodovano tkivo, preveriti njegovo učinkovitost v procesu rehabilitacije ter določiti smernice in parametre za uspešno izvajanje. Literaturo smo iskali v šestih podatkovnih bazah: Cobiss, PubMed, PEDro, Cochrane Library, Google Učenjak in Science Direct, za obdobje med letoma 2004 in 2024, na osnovi naslednjih ključnih besed: »podaljšano raztezanje«, »fizioterapija«, »prolonged«, »stretch«, »prolonged stretch«, »prolonged static stretch«, »long duration stretch«, »physiotherapy«, »rehabilitation«, »injury«. Potek pridobivanja in izbiranja literature smo predstavili v diagramu PRISMA, njeno kakovost smo ocenili z lestvico PEDro, pridobljene podatke pa nato kvalitativno obdelali in analizirali. Izmed 1928 pridobljenih je bilo v končno analizo vključenih 5 študij s povprečno metodološko kakovostjo 5,3/10,0, ki so vključevale paciente z zmanjšanim obsegom giba zaradi kontraktur. Obseg giba se je s pomočjo PSR značilno izboljšal v 4 od 5 analiziranih študij. Parametri za uspešno raztezanje so bili naslednji: trajanje 10–60 min, intenzivnost nizka–visoka, pogostost 3x–7x/teden (tudi večkrat dnevno), za vsaj dva tedna ali do doseţenega platoja v obsegu giba. Ugotavljamo, da PSR vpliva na biomehanske lastnosti tkiv, kot sta plastičnost in viskoznost ter na avtogeno inhibicijo miotatičnega refleksa in Golgijevega kitnega organa, kar omogoča trajno izboljšanje gibljivosti in razteznosti tkiv. Učinkovitost te tehnike je še posebej izrazita pri pacientih, kjer druge oblike raztezanja ne zadostujejo, kot so kontrakture in adhezivni kapsulitis.

Abstract:Prolonged static stretching (PSS) is one of the techniques used to improve flexibility. However its effectiveness in rehabilitation and impact on injured tissue are less known. The aim of this thesis was to systematically review literature to determine PSS effects on injured tissue, evaluate its effectiveness in the rehabilitation process, and define guidelines and parameters for effective application. Literature was searched in six databases: Cobiss, PubMed, PEDro, Cochrane Library, Google Scholar, and Science Direct, covering the period 2004–2024, using the following keywords: »podaljšano raztezanje«, »fizioterapija«, »prolonged«, »stretch«, »prolonged stretch«, »prolonged static stretch«, »long duration stretch«, »physiotherapy«, »rehabilitation«, »injury«. The process of selection is presented using the PRISMA diagram. Study quality was assessed using the PEDro scale, and the data were qualitatively analyzed. Out of 1,928 records, 5 studies with an average methodological quality score of 5.3/10.0 and involving patients with reduced range of motion due to contracture were included. In 4 of 5 studies, PSS significantly improved range of motion. Parameters for effective stretching included 10–60 minutes duration, low to high intensity, frequency of 3–7 times per week (even multiple times per day), for at least two weeks or until a plateau in range of motion occurred. PSS influences biomechanical tissue properties such as plasticity and viscosity, and facilitates autogenic inhibition of the stretch reflex and Golgi tendon organ, enabling lasting improvements in flexibility and tissue extensibility. Its effectiveness is particularly evident in cases where other stretching methods fall short, such as contractures and adhesive capsulitis.
