Title:Fizioterapevtska obravnava bolnika s sindromom zamrznjene rame – pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje

Abstract:Teoretična izhodišča: Sindrom zamrznjene rame (ZR) je bolezensko stanje, za katero sta značilna bolečina in omejena gibljivost v ramenskem sklepu. Zaradi zapletene patofiziologije, ki vključuje vnetne in fibrozne spremembe sklepne ovojnice, fizioterapevtska obravnava predstavlja temeljni pristop za zmanjšanje simptomov in izboljšanje funkcije. Metoda: Izveden je bil pregled literature, vključno z devetimi študijami, ki so izpolnjevale vključitvene kriterije. Analizirane so bile randomizirane kontrolirane raziskave z oceno ≥ 6/10 na lestvici PEDro. Podatki so bili obdelani s tematsko analizo, pri čemer so bile oblikovane kategorije glede na uporabljene terapevtske pristope. Rezultati: Ugotovljeno je bilo, da manualne mobilizacije, vadbeni programi in kombinirani pristopi pomembno pripomorejo k zmanjšanju bolečine in izboljšanju obsega gibljivosti ter funkcionalnosti pri bolnikih s sindromom ZR. Multimodalni programi, ki združujejo več terapij, so pokazali najobsežnejše izboljšave. Med vsemi pristopi pa se je kot najbolj učinkovit izkazal kombiniran pristop fizioterapije z uporabo kontratrakcijskega aparata za inferiorno raztezanje kapsule rame. Razprava: Kljub pozitivnim učinkom obravnav fizioterapije so bile zaznane določene omejitve, vključno z različnimi protokoli obravnave, kratkotrajnim spremljanjem in heterogenostjo uporabljenih merskih instrumentov. Nadaljnje raziskave so potrebne za standardizacijo pristopov in dolgoročno oceno terapevtskih učinkov.

Abstract:Theoretical background: Frozen shoulder syndrome (FS) is a pathological condition characterized by pain and limited mobility of the shoulder joint. Due to its complex pathophysiology, involving inflammatory and fibrotic changes of the joint capsule, physiotherapy remains a cornerstone in symptom management and functional recovery. Method: A literature review was conducted, including nine studies that met the inclusion criteria. Only randomized controlled trials with a PEDro score of ≥ 6/10 were analysed. The data were processed using thematic analysis, with categories based on the therapeutic approaches used. Results: It was found that manual mobilizations, exercise programs, and combined approaches significantly contributed to pain reduction and improvements in range of motion and functionality in patients with FS syndrome. Multimodal programs combining several therapies demonstrated the most extensive improvements. Among all approaches, the combined physiotherapy approach using a countertraction device for inferior capsular stretching proved to be the most effective. Discussion: Despite the positive effects of physiotherapy interventions, certain limitations were noted, including variability in treatment protocols, short follow-up periods, and heterogeneity of outcome measures. Further research is needed to standardize approaches and assess the long-term effects of therapies.
