Abstract:Uvod: Poškodba hrbtenjače v torakalnem delu hrbtenice pogosto vodi v zmanjšano mišično moč, motnje ravnotežja in oteženo hojo, zato je izbira učinkovitih fizioterapevtskih pristopov ključna za izboljšanje funkcionalnih sposobnosti in kakovosti življenja posameznika. Cilji: Raziskati in opredeliti najučinkovitejše pristope pri obravnavi poškodb hrbtenjače v torakalnem delu hrbtenice ter jih prenesti v prikaz primera. Metode: Izveden je bil pregled sedmih randomiziranih raziskav (PEDro ≥ 6), ki so preučevale krepilno vadbo, izboljšanje ravnotežja in vadbo hoje, vključno s tehnologijami povratne informacije. Postopek iskanja in selekcije raziskav je bil prikazan s pomočjo PRISMA diagrama. Rezultati: Ciljno usmerjeni, progresivni programi in programi, usmerjeni v specifične naloge (progresivna vadba moči, krožni trening, FES-kolesarjenje, napredne vaje na blazini, VR-vadba in hoja z vizualno ter zvočno povratno informacijo o obremenitvi) so dosledno izboljšali mišično moč in vzdržljivost, dinamično ravnotežje, hitrost hoje ter funkcionalno sposobnost. Posebej učinkovite so se izkazale intervencije z neposredno in merljivo povratno informacijo o prenosu teže na spodnje okončine. V prikazu primera so se kratkoročno izboljšali ključni kazalniki mobilnosti: hitrost hoje se je povečala z 0,45 m/s na 0,65 m/s, razdalja v šestminutnem testu hoje pa z 170 m na 205 m. Zaključek: Optimalna obravnava temelji na individualizirani kombinaciji progresivnih, ponovljivih in funkcionalno usmerjenih vaj, dopolnjenih s sprotno povratno informacijo ter vključevanjem konvencionalnih fizioterapevtskih tehnik. Kljub spodbudnim izsledkom so za potrditev ugotovitev potrebne metodološko strožje in dolgoročnejše raziskave o učinkih fizioterapije pri obravnavi oseb s poškodbo hrbtenjače.

Abstract:Introduction: Thoracic spinal cord injury often leads to reduced muscle strength, impaired balance, and difficulties in walking, making the choice of effective physiotherapy approaches essential for improving functional abilities and quality of life. Objectives: To identify the most effective approaches in the management of thoracic spinal cord injury and apply them in a case study. Methods: A review of seven randomized trials (PEDro ≥ 6) was conducted, focusing on strengthening exercises, balance training, and gait training, including feedback-based technologies. The search and selection process was presented using a PRISMA diagram. Results: Targeted, progressive, and task-specific programs (progressive strength training, circuit training, FES-cycling, advanced mat exercises, VR-based training, and gait training with visual and auditory weight-bearing feedback) consistently improved muscle strength and endurance, dynamic balance, walking speed, and functional capacity. Interventions providing direct and measurable feedback on weight transfer to the lower limbs proved particularly effective. In the case study, short-term improvements were observed in key mobility indicators: walking speed increased from 0.45 m/s to 0.65 m/s, and the 6-minute walking distance improved from 170 m to 205 m. Conclusion: Optimal rehabilitation is based on an individualized combination of progressive, repetitive, and functionally oriented exercises, complemented by real-time feedback and conventional physiotherapy techniques. Despite promising results, methodologically stronger and longer-term studies are needed to confirm the effectiveness of physiotherapy interventions in individuals with spinal cord injury.
