Abstract:Teoretična izhodišča: Fantomska bolečina je najpogostejši bolečinski pojav po amputaciji uda. Gre za bolečino, ki se pojavlja v manjkajočem delu uda in omejuje posameznikov vsakdan. Ker vključuje mehanizme na različnih ravneh, natančen izvor pa še ni povsem pojasnjen, je njeno učinkovito zdravljenje omejeno. Fizioterapija predstavlja ključen del rehabilitacije, saj omogoča zmanjšanje fantomske bolečine, izboljšanje funkcionalnosti in ponovno vključevanje v vsakdanje aktivnosti. Namen: Raziskati najpogosteje uporabljene in najučinkovitejše fizioterapevtske pristope pri zmanjšanju fantomske bolečine po amputaciji spodnjega uda ter posebej ugotoviti učinek terapije z ogledalom. Metode dela: Metodologija diplomskega dela je kvalitativna. Uporabili smo deskriptivno raziskovalno metodo in metodo kompilacije ter komparacije podatkov. V empirični del diplomskega dela smo vključili devet raziskav, ki so bile izvedene med letoma 2014 in 2024. Raziskave so bile izbrane na podlagi vključitvenih in izključitvenih kriterijev. Za prikaz izbire pregledane literature smo uporabili diagram PRISMA. Rezultati: Ugotovili smo, da se uporabljajo različni fizioterapevtski pristopi pri zdravljenju fantomske bolečine po amputaciji spodnjega uda. Najpogosteje se uporablja terapija z ogledalom, ki je tudi najučinkovitejši pristop, prav tako je učinkovito kombiniranje različnih metod med seboj v primerjavi z zgolj izvajanjem klasične fizioterapije. Ne obstaja točno določen protokol, ki bi pri vsakemu posamezniku enako vplival na zmanjšanje fantomske bolečine. Terapija z ogledalom vpliva na zmanjšanje intenzivnosti fantomske bolečine, njena učinkovitost je dolgoročna z rednim izvajanjem. Razprava: Multimodalen pristop je v primerjavi s samostojnim izvajanjem bolj smiseln in učinkovit pri zdravljenju fantomske bolečine po amputaciji spodnjega uda. Raznolikost parametrov fantomske bolečine pri posameznikih opozarja na individualno prilagoditev protokola zdravljenja in neuspešnost univerzalnega pristopa.

Abstract:Background: Phantom pain is the most common pain phenomenon after limb amputation. It is pain that occurs in the missing part of the limb and limits the individual's daily life. Since it involves mechanisms at different levels and the exact origin is not yet fully understood, its effective treatment is limited. Physiotherapy is a key part of rehabilitation, as it allows for the reduction of phantom pain, improvement of functionality and reintegration into daily activities. Aim: Investigate the most commonly used and the most effective physiotherapeutic approaches for reducing phantom pain after lower limb amputation and specifically determine the effect of mirror therapy. Methods: The methodology of the thesis is qualitative. A descriptive research method was used, along with compilation and comparison of data. The empirical part includes nine studies conducted between 2014 and 2024, selected based on inclusion and exclusion criteria. The PRISMA diagram was used to illustrate the selection process of the reviewed literature. Results: Various physiotherapy approaches are used in the treatment of phantom pain after lower limb amputation. Mirror therapy is the most frequently applied method, which is also the most effective approach, and combining different methods has also proven to be more effective than conventional physiotherapy alone. Therefore, there is no specific protocol that would have the same effect on reducing phantom pain in every individual. Mirror therapy contributes to the reduction of pain intensity and shows long-term effectiveness when performed regularly. Discussion: A multimodal approach is more reasonable and effective compared to single-method interventions in the treatment of phantom pain after lower limb amputation. The variability of phantom pain parameters among individuals highlights the need for individualized treatment protocols and the limitations of a universal approach.
