Title:Učinkovitost terapije miofascialnih prožilnih točk pri mialgiji zgornjega dela m. trapeziusa – pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Teoretična izhodišča: Miofascialne prožilne točke (MPT) so pogost vzrok mišično-skeletne bolečine, zlasti v zgornjem delu m. trapezius. Njihovo zdravljenje vključuje širok spekter terapevtskih pristopov, od manualnih tehnik do invazivnih procedur. Namen tega pregleda literature je bil ovrednotiti učinkovitost različnih terapij MPT na tem področju. Metoda: Uporabljena je bila metoda sistematičnega pregleda literature. Iskanje po ključnih besedah “myofascial trigger points”, “upper trapezius myalgia”, “upper trapezius” in “trigger point therapy" je bilo izvedeno v elektronskih podatkovnih bazah Google Scholar, ScienceDirect, PubMed, ProQuest, Scopus, PEDro za obdobje 2009–2024. Vključene so bile randomizirane kontrolirane raziskave v angleškem jeziku. Rezultati: V končno analizo je bilo vključenih 10 raziskav. Rezultati kažejo, da različne terapije (manualne tehnike, suho iglanje, elektroterapija) statistično zmanjšajo bolečino in izboljšajo funkcionalno stanje v kratkoročnem obdobju. Vendar med njimi ni bilo ugotovljenih bistvenih razlik v učinkovitosti. Razprava: Kljub dokazom o učinkovitosti številnih terapij pregled izpostavlja pomanjkanje standardiziranih diagnostičnih protokolov in objektivnih merilnih instrumentov. Pomanjkljivost je tudi odsotnost merjenja biomehanskih parametrov, kot je mišična togost. Prihodnje raziskave bi se morale osredotočiti na razvoj objektivnejših diagnostičnih meril in dolgoročno spremljanje učinkov.

Abstract:Theoretical background: Myofascial trigger points (MTrPs) are a common cause of musculoskeletal pain, particularly in the upper trapezius muscle. Their treatment encompasses a wide range of therapeutic approaches, from manual techniques to invasive procedures. The purpose of this literature review was to evaluate the effectiveness of various MTrP therapies in this region. Methods: A systematic literature review methodology was employed. A keyword search “myofascial trigger points”, “upper trapezius myalgia”, “upper trapezius” in “trigger point therapy" was conducted in electronic databases Google Scholar, ScienceDirect, PubMed, ProQuest, Scopus, PEDro for the period 2009–2024. Randomized controlled trials (RCTs) in English were included. Results: Ten studies were included in the final analysis. Results indicate that various therapies (manual techniques, dry needling, electrotherapy) significantly reduce pain and improve functional status in the short term. However, no substantial differences in efficacy were found between them. Discussion: Despite evidence supporting the effectiveness of numerous therapies, the review highlights a lack of standardized diagnostic protocols and objective measurement tools. A significant shortcoming is the absence of biomechanical parameter measurement, such as muscle stiffness. Future research should focus on developing more objective diagnostic criteria and implementing long-term outcome monitoring.
