Title:Vloga medicinske sestre pri obravnavi debelosti pri otrocih in mladostnikih: sistematični pregled literature

Abstract:Teoretična izhodišča: Debelost pri otrocih in mladostnikih predstavlja pomemben javnozdravstveni problem, ki se povezuje s številnimi telesnimi, psihološkimi in socialnimi posledicami ter pomembno vpliva na kakovost življenja otrok in njihovih družin. Kljub vse večji razširjenosti debelosti je obravnava pogosto preozko usmerjena, brez zadostnega upoštevanja psihosocialnih in okoljskih dejavnikov, kar lahko vodi v stigmatizacijo in manj učinkovito obravnavo otrok in mladostnikov. Namen magistrske naloge je bil s sistematičnim pregledom literature sintezno opredeliti vlogo medicinske sestre pri obravnavi otrok in mladostnikov z debelostjo, prepoznati ključne dejavnike uspešne obravnave ter izpostaviti priložnosti za izboljšanje prakse in nadaljnje raziskovanje. Metode: Raziskava je temeljila na sistematičnem pregledu strokovne in znanstvene literature, izvedenem skladno s smernicami PRISMA. V pregled in analizo je bilo vključenih 22 znanstvenih raziskav s področja obravnave debelosti pri otrocih in mladostnikih. Pred poglobljeno analizo vsebine posameznih raziskav smo zbrali njihove osnovne značilnosti ter ocenili metodološko kakovost in tveganje pristranskosti z uporabo ustreznih ocenjevalnih orodij glede na zasnovo posamezne študije. Podatke smo analizirali s kvalitativno tematsko analizo, pri čemer smo izvedli kvalitativno sintezo ugotovitev s kodiranjem vsebin in njihovim združevanjem v vsebinske kategorije. Rezultati: Analiza vključenih raziskav je pokazala, da debelost pomembno vpliva na telesno zdravje, psihološko počutje, samopodobo, socialne odnose in šolsko delovanje otrok in mladostnikov. Ugotovljeno je bilo, da ima medicinska sestra osrednjo vlogo pri zdravstveni vzgoji, preventivnih dejavnostih, motivacijskem svetovanju ter podpori otrokom in njihovim družinam. Pomembni dejavniki uspešne obravnave so vključevanje družine, dostopnost programov in podpora okolja. Razprava: Ugotovitve poudarjajo potrebo po celostnem in multidisciplinarnem pristopu k obravnavi debelosti, v katerem ima medicinska sestra pomembno vlogo pri podpori otrokom in njihovim družinam, zlasti pri zmanjševanju stigme, spodbujanju zdravega življenjskega sloga ter dolgoročnem spremljanju otrok in mladostnikov. Izpostavljena je tudi potreba po večji vključenosti vzgojno-izobraževalnih ustanov, boljši dostopnosti preventivnih programov in ustvarjanju okolja, ki omogoča zdrave izbire, kar lahko prispeva k izboljšanju kakovosti življenja otrok in mladostnikov z debelostjo ter k učinkovitejšemu preprečevanju in obravnavi tega problema.

Title:The role of nurse in the management of obesity in children and adolescents: a systematic review of the literature
Abstract:Theoretical starting points: Childhood and adolescent obesity represent a significant public health problem associated with numerous physical, psychological and social consequences, substantially affecting the quality of life of children and their families. Despite the increasing prevalence of obesity, its management is often narrowly focused and insufficiently considers psychosocial and environmental factors, which may contribute to stigmatization and less effective care. The aim of this master's thesis was to synthesize the role of nurses in the management in children and adolescents with obesity through a systematic review of the literature, to identify key factors contributing to successful management, and to highlight opportunities for improving practice and future research. Methods: The research was based on a systematic review of professional and scientific literature conducted in accordance with the PRISMA guidelines. A total of 22 scientific studies focusing on the management of obesity in children and adolescents were included in the analysis. Prior to the in-depth content analysis, the basic characteristics of the included studies were identified, and their methodological quality and risk of bias were assessed using appropriate appraisal tools according to the study design. The data were analyzed using thematic qualitative analysis and qualitative data synthesis was performed by coding content and subsequently grouping it into thematic categories. Results: The analysis of the included studies showed that obesity significantly affects physical health, psychological well-being, self-esteem, social relationships and school functioning of children and adolescents. The findings indicate that nurses play a key role in health education, preventive activities, motivational counselling and in providing support to children and their families. Key factors contributing to successful obesity management include family involvement, accessibility of programmes and supportive environmental conditions. Discussion: The findings highlight the need for a comprehensive and multidisciplinary approach to the management of children and adolescents with obesity, in which nurses have an important role in supporting children and their families, particularly in reducing stigma, promoting healthy lifestyles and providing long-term follow-up. The results also indicate the importance of greater involvement of educational institutions, improved accessibility of preventive programmes and the creation of environments that enable healthy choices, which may contribute to improving the quality of life of children and adolescents with obesity and to more effective prevention and management of this condition.
