Title:Prvi travmatski izpah ramenskega sklepa: retrospektivna študija : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Glenohumeralni (GH) oziroma ramenski sklep je najbolj gibljiv sklep človeškega telesa. Anatomija sklepa omogoča velik obseg gibanja, vendar hkrati povečuje tveganje za izpah in razvoj nestabilnosti. Ramenski sklep je namreč najpogosteje izpahnjen sklep človeškega telesa, pri čemer sprednji izpah ramenskega sklepa (SIRS) predstavlja večino izpahov. Namen diplomskega dela je ugotoviti vpliv starosti ob prvem travmatskem SIRS na vrsto poškodb anatomskih struktur ter tveganje za ponovni izpah. Preučili smo tudi čas imobilizacije ter priporočeno gibanje med in po imobilizaciji. Metodologija: V teoretičnem delu smo z deskriptivno metodo predstavili dosedanja spoznanja o SIRS. V empiričnem delu smo izvedli retrospektivno kvantitativno študijo. Podatke smo pridobili iz zdravniške dokumentacije Univerzitetnega kliničnega centra Ljubljana. Vzorec je zajemal 78 bolnikov, ki so leta 2023 doživeli prvi travmatski SIRS. Rezultati: Poškodbe rotatorne manšete so značilne pri starejših bolnikih (> 40 let). Z vidika kostnih poškodb je bila Hill-Sachs lezija pogostejša med mlajšimi bolniki (≤ 40 let), zlom velikega tuberkla pa med starejšimi (> 40 let). Vpliv starosti na pojavnost ponovnega izpaha nismo ugotovili. Z vidika konzervativne obravnave smo zabeležili, da je imobilizacija v večini primerov trajala 3 tedne ali manj. Med imobilizacijo je bilo indicirano gibanje distalnih sklepov poškodovanega uda, gibanje GH sklepa pa v večini primerov po zaključku imobilizacije. Razprava: Na podlagi rezultatov ugotavljamo, da ima starost pomemben vpliv na vrsto poškodb pri prvem travmatskem SIRS. Konzervativna obravnava sledi sodobnim smernicam krajše imobilizacije. Indicirano gibanje v zgodnji fazi rehabilitacije je izvedeno znotraj varnega obsega gibanja in položajev, ki ne zmanjšujejo stabilnosti in ne obremenjujejo poškodovanih struktur.

Abstract:The glenohumeral (GH) or shoulder joint is the most mobile joint in the human body. The anatomy of the joint enables a large range of motion but simultaneously increases the risk of dislocation and the development of instability. The shoulder joint is, in fact, the most frequently dislocated joint in the human body, with anterior shoulder dislocation (ASD) accounting for the majority of dislocations. The aim of thesis is to determine the influence of age at the time of the first traumatic ASD on the types of injuries to anatomical structures and the risk of recurrent dislocation. We also examined the duration of immobilization and the recommended movement during and after immobilization. Methodology: In the theoretical part, we presented existing knowledge on ASD using a descriptive method. In the empirical part, we conducted a retrospective quantitative study. Data were obtained from the medical records of the University Medical Centre Ljubljana. The sample consisted of 78 patients who experienced their first traumatic ASD in 2023. Results: Rotator cuff injuries were characteristic of older patients (> 40 years). From the perspective of bony injuries, Hill-Sachs lesions occurred more frequently in younger patients (≤ 40 years), whereas fractures of the greater tubercle of the humerus were more frequent in older patients (> 40 years). We found no effect of age on the incidence of recurrent dislocation. Regarding conservative management, immobilization lasted for 3 weeks or less in most cases. During immobilization, movement of the distal joints of the affected limb was recommended, whereas GH joint movement was mostly initiated after the end of immobilization. Discussion: Based on the results, we conclude that age has an important influence on the types of injuries in the first traumatic ASD. Conservative management follows modern guidelines of shorter immobilization. Recommended movement in the early phase of rehabilitation is performed within a safe range of motion and positions that do not compromise stability or stress the injured structures.
