Title:Primerjava vaj v odprti kinetični verigi in zaprti kinetični verigi na izboljšanje funkcionalnosti kolena po rekonstrukciji sprednje križne vezi : pregled literature

Abstract:Teoretična izhodišča: Poškodba sprednje križne vezi je ena izmed najpogostejših poškodb kolena. Pojavlja se predvsem pri športnih aktivnostih, ki vključujejo pivotiranje, skoke in nenadne spremembe smeri. V rehabilitaciji po rekonstrukciji sprednje križne vezi se uporabljajo vaje v odprti in zaprti kinetični verigi. Vaje v odprti kinetični verigi omogočajo izolirano aktivacijo mišic in izboljšanje gibljivosti sklepa, medtem ko vaje v zaprti kinetični verigi aktivirajo več mišičnih skupin, izboljšajo stabilnost in funkcionalni nadzor kolena. Namen: Namen diplomskega dela je bil preučiti in primerjati vpliv vaj v odprti in zaprti kinetični verigi na izboljšanje funkcionalnosti kolena po rekonstrukciji sprednje križne vezi. Metode: Uporabljena je kvalitativna metodologija. V teoretičnem delu smo predstavili anatomijo kolena, mehanizme poškodb sprednje križne vezi in značilnosti vaj v zaprti kinetični verigi ter odprti kinetični verigi. V empiričnem delu smo izvedli kvalitativno analizo strokovnih raziskav, sistematičnih pregledov in metaanaliz. Rezultati: V natančno analizo smo zajeli osem strokovnih člankov, ki so preučevali vpliv vaj v odprti in zaprti kinetični verigi na obseg giba, mišično moč, bolečino in funkcionalnost kolena. Vaje v zaprti kinetični verigi so bolj učinkovite pri zmanjšanju bolečine, povečanju obsega giba, mišične moči in funkcionalnosti kolena. Vaje v odprti kinetični verigi izboljšajo omenjene parametre v manjši meri, omogočajo pa dodatno izolirano krepitev mišic. Razprava: Obe vrsti vaj pozitivno vplivata na izboljšanje funkcionalnosti kolena. Kombinacija vaj v odprti in zaprti kinetični verigi omogoča najbolj optimalne rezultate po rekonstrukciji sprednje križne vezi.

Abstract:Theoretical background: Anterior cruciate ligament injury is one of the most common knee injuries. It occurs mainly during sports activities that involve pivoting, jumping, and sudden changes of direction. Open and closed kinetic chain exercises are commonly used in rehabilitation following anterior cruciate ligament reconstruction. Open kinetic chain exercises promote isolated muscle activation and improve joint mobility, while closed kinetic chain exercises activate multiple muscle groups, enhancing knee stability and functional control. Purpose: The purpose of this thesis was to examine and compare the effects of open and closed kinetic chain exercises on improving knee function following anterior cruciate ligament reconstruction. Methods: A qualitative methodology was used. The theoretical part presents the anatomy of the knee, the mechanisms of anterior cruciate ligament injuries, and the characteristics of closed and open kinetic chain exercises. In the empirical part, we performed a qualitative analysis of scientific studies, systematic reviews, and meta-analyses. Results: We included eight scientific articles in our detailed analysis, which examined the effects of open and closed kinetic chain exercises on range of motion, muscle strength, pain, and knee function. Closed kinetic chain exercises were found to be more effective in reducing pain and increasing range of motion, muscle strength, and knee function. Open kinetic chain exercises improved these parameters to a lesser extent but allowed for additional isolated muscle activation. Discussion: Both types of exercises have a positive effect on improving knee function. A combination of open and closed kinetic chain exercises provides the most optimal results following anterior cruciate ligament reconstruction.
