Title:Fizioterapevtski pristopi pri subluksaciji ramenskega sklepa pri pacientih po možganski kapi : pregled literature

Abstract:Možganska kap predstavlja enega najpogostejših vzrokov trajne invalidnosti in smrti ter prinaša številne gibalne in funkcionalne posledice, ki vplivajo na vsakdanje življenje pacientov. Med zapleti, ki se pogosto pojavijo po možganski kapi, je subluksacija ramenskega sklepa, ki otežuje rehabilitacijo zgornje okončine in zmanjšuje možnost za funkcionalno okrevanje. Diplomsko delo je usmerjeno v preučevanje fizioterapevtskih pristopov pri obravnavi tega zapleta. Teoretični del zajema opredelitev možganske kapi, njene vrste, etiološke dejavnike, epidemiologijo, patofiziološke mehanizme, klinično sliko in diagnostične postopke. Posebej so opisani anatomska in biomehanska zgradba ramenskega sklepa, spremembe, ki nastanejo v ramenskem sklepu po možganski kapi ter značilnosti in potek subluksacije. V empiričnem delu je bil opravljen pregled znanstvene in strokovne literature. Analiza je prikazala najpogosteje uporabljene fizioterapevtske pristope za preprečevaje in zdravljenje subluksacije ramenskega sklepa ter njihov učinek na dolgoročne funkcionalne izide pacientov po možganski kapi. Ugotovitve različnih avtorjev potrjujejo, da pravočasno prepoznavanje in ustrezna obravnava pacientov bistveno prispevata k izboljšanju funkcionalne zmogljivosti, zmanjšanju zapletov ter višji kakovosti življenja pacientov po možganski kapi. Diplomsko delo hkrati opozarja na potrebo po nadaljnjih raziskavah in pripravi standardiziranih, z dokazi podprtih protokolov za obravnavo bolnikov s subluksacijo ramenskega sklepa.

Abstract:Stroke represents one of the most common causes of permanent disability and mortality and leads to numerous motor and functional consequences that significantly affect the patient’s daily lives. Among the complications frequently occurring after stroke is shoulder joint subluxation, which hinders upper limb rehabilitation and reduces the potential for functional recovery. The thesis focuses on examining physiotherapeutic approaches in the management of this complication. The theoretical part provides a comprehensive overview of stroke, it’s types, etiological factors, epidemiology, pathophysiological mechanisms, clinical presentation and diagnostic procedures. Particular emphasis is placed on the anatomical and biomechanical structure of the shoulder joint, the changes that occur after stroke and the characteristics and progression of shoulder subluxation. The empirical part includes a review of scientific and professional literature. The analysis identified the most commonly used physiotherapeutic methods for the prevention and treatment of shoulder subluxation and evaluated their effects on the long-term functional outcomes of patients after stroke.Findings from various authors confirm that timely recognition and appropriate physiotherapeutic intervention significantly contribute to improving functional capacity, reducing complications and enhancing the quality of life of post-stroke patients. The thesis also highlights the need for further research and the development of standardized, evidence-based protocols for the management of patients with shoulder subluxation following stroke.
