Title:Preprečevanje padcev v bolnišnici pri starostnikih z arterijsko hipertenzijo

Abstract:Teoretična izhodišča: Padci predstavljajo enega najpogostejših neželenih dogodkov pri starostnikih v bolnišničnem okolju in pomembno vplivajo na njihovo varnost, zdravstveno stanje ter kakovost življenja. Starostniki z arterijsko hipertenzijo so zaradi starostnih sprememb, pridruženih kroničnih bolezni, polifarmacije in vpliva antihipertenzivne terapije še posebej izpostavljeni tveganju za padec. Preprečevanje padcev temelji na pravočasnem prepoznavanju dejavnikov tveganja, sistematičnem ocenjevanju tveganja, izvajanju ustreznih preventivnih ukrepov ter izobraževanju pacientov in njihovih svojcev. Pomembno vlogo pri tem ima medicinska sestra, ki s svojim strokovnim znanjem in kontinuirno prisotnostjo pomembno prispeva k zagotavljanju varnosti hospitaliziranih starostnikov. Namen raziskave je bil preučiti preprečevanje padcev pri starostnikih z arterijsko hipertenzijo v bolnišničnem okolju ter osvetliti vlogo zaposlenih v zdravstveni negi pri tem procesu. Metoda: Raziskava je temeljila na kvalitativnem raziskovalnem pristopu z uporabo deskriptivne metode. Empirični del je bil izveden s polstrukturiranimi intervjuji. V raziskavi je sodelovalo 20 zaposlenih v zdravstveni negi v bolnišničnem okolju, in sicer 10 diplomiranih medicinskih sester in 10 srednjih medicinskih sester. Zbrani podatki so bili analizirani s kvalitativno vsebinsko analizo. Rezultati: Intervjuvanci so kot najpogostejše dejavnike tveganja za padce pri starostnikih z arterijsko hipertenzijo navedli ortostatsko hipotenzijo, motnje ravnotežja in hoje, mišično oslabelost, kognitivne motnje ter vpliv antihipertenzivne in druge sočasne terapije. Ocenjevanje tveganja najpogosteje vključuje uporabo standardiziranih ocenjevalnih lestvic, dopolnjenih s klinično presojo. Preventivni ukrepi obsegajo prilagoditve bolnišničnega okolja, nadzor in spremstvo pri gibanju, interdisciplinarno sodelovanje ter izobraževanje pacientov in njihovih svojcev. Razprava: Ugotovitve kažejo, da zaposleni v zdravstveni negi dobro prepoznavajo dejavnike tveganja za padce in v klinični praksi izvajajo preventivne ukrepe, ki so v veliki meri skladni s strokovnimi priporočili. Kljub temu se pri njihovem izvajanju pojavljajo organizacijske in kadrovske omejitve, ki lahko vplivajo na doslednost preventive. Izobraževanje pacientov in njihovih svojcev je prepoznano kot pomemben del preprečevanja padcev, vendar je v praksi pogosto časovno omejeno. Vloga medicinske sestre se je izkazala kot ključna pri zagotavljanju varnosti starostnikov z arterijsko hipertenzijo v bolnišničnem okolju.

Title:Prevention off Falls among Hospitalized Elderly Patients Eith Arterial Hypertension
Abstract:Theoretical background: Falls are among the most common adverse events affecting older adults in hospital settings and have a significant impact on patient safety, health status, and quality of life. Older adults with arterial hypertension are particularly vulnerable to falls due to age-related physiological changes, comorbidities, polypharmacy, and the effects of antihypertensive therapy. Fall prevention is based on early identification of risk factors, systematic fall risk assessment, implementation of appropriate preventive measures, and education of patients and their relatives. Nurses play a key role in this process, as their professional knowledge and continuous presence significantly contribute to ensuring patient safety. The aim of this study was to examine fall prevention in older adults with arterial hypertension in a hospital setting and to highlight the role of nursing staff in this process. Methods: The study was based on a qualitative research approach using a descriptive method. The empirical part of the research was conducted using semi-structured interviews. The sample consisted of 20 nursing professionals employed in a hospital setting, including 10 registered nurses and 10 practical nurses. The collected data were analyzed using qualitative content analysis. Results: The participants identified orthostatic hypotension, impaired balance and gait, muscle weakness, cognitive impairment, and the effects of antihypertensive and other concomitant medications as the most common risk factors for falls in older adults with arterial hypertension. Standardized fall risk assessment tools were most frequently used and were supplemented by clinical judgment. Preventive measures included environmental adjustments, supervision and assistance during mobilization, interdisciplinary collaboration, and education of patients and their relatives. Discussion: The findings indicate that nursing staff are able to recognize fall risk factors and implement preventive measures that are largely consistent with professional recommendations. However, organizational and staffing limitations often affect the consistency of preventive practices. Patient and family education is recognized as an important component of fall prevention, although it is frequently limited by time constraints in clinical practice. The role of nurses proved to be crucial in ensuring the safety of older adults with arterial hypertension in hospital settings.
