Title:Fizioterapevtski pristopi pri odraslih pacientih s cerebralno paralizo – pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Teoretična izhodišča: Pri cerebralni paralizi pride do poškodbe možganov pred porodom, med ali po porodu. Je neprogresivna nevrološka motnja, ki zajema motnje gibanja, drže, ravnotežja in motoričnih funkcij. Več kot 80 % oseb s CP, ki preživi do 20. leta, preživi tudi do 50. leta starosti. Odrasli s CP se soočajo s sekundarnimi težavami, ki se razlikujejo glede na obliko in stopnjo CP. Pri teh stanjih ima fizioterapija osrednjo vlogo, saj lahko skupaj s fizično aktivnostjo zamakne upad mobilnosti. Metode: V diplomskem delu smo s pregledom osmih člankov, ki smo jih vključili v naše delo, odgovorili na dve raziskovalni vprašanji. Članke smo iskali v bazah PubMed, Google Scholar in ScienceDirect. Rezultati: S končno analizo vključenih raziskav smo ugotovili, da so fizioterapevtski pristopi pri odraslih pacientih s CP učinkoviti pri izboljšanju kinematike hoje, mišične moči, hitrosti in vzdržljivosti hoje ter respiratornih parametrov. Razprava: Pri odraslih osebah s CP se pogosteje pojavljata dva fizioterapevtska pristopa, in sicer trening hoje na tekalni stezi ter vadba za moč. Učinkovita sta pri izboljšanju mišične moči, kinematike hoje, vzdržljivosti in hitrosti hoje. Tudi ostali fizioterapevtski pristopi so pokazali učinkovitost, vendar so študije, ki to potrjujejo nizke metodološke kakovosti. Slednja omejitev na tem področju nakazuje na potrebo po nadaljnem raziskovanju učinkovitosti fizioterapevtskih pristopov pri odrasli populaciji s CP.

Abstract:Theoretical background: In cerebral palsy (CP), brain injury occurs before birth, during birth, or shortly after birth. It is a non-progressive neurological disorder characterized by impairments in movement, posture, balance, and motor functions. More than 80% of individuals with CP who survive to the age of 20 also live to the age of 50. Adults with CP face secondary conditions that vary depending on the type and severity of CP. In these conditions, physiotherapy plays a central role, as together with physical activity it can delay the decline in mobility. Methods: In the thesis, we addressed two research questions through a review of eight articles included in our study. The articles were searched in the PubMed, Google Scholar, and ScienceDirect databases. Results: The final analysis of the included studies showed that physiotherapeutic approaches in adult patients with CP can be effective in improving gait kinematics, muscle strength, gait speed, gait endurance, and respiratory parameters. Discussion: In adults wth CP, two physiotherapy approaches appear more frequently, namely treadmill gait training and strenght training.These approaches are effective for improving muscle strength, gait kinematics, endurance, and speed. Other physiotherapeutic approaches have also demonstrated effectiveness, however, the studies supporting these findings are of low methodological quality. This limitation, in the field indicates a need for further research on the effectiveness of physiotherapeutic approaches in the adult population with CP.
