Title:Učinek nevrofizioterapevtskih tehnik v kombinaciji z refleksno lokomocijo na variabilnost srčne frekvence pri otroku s cerebralno paralizo: študija primera : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Uvod: Cerebralna paraliza je najpogostejša telesna oviranost v otroštvu in je posledica poškodbe ali nenormalnega razvoja možganov. Zgodnja postavitev diagnoze je ključna, saj otroku omogoča čimprejšnjo vključitev v ustrezne terapevtske intervencije. Adekvatne fizioterapevtske obravnave otrok s cerebralno paralizo vključujejo efektivne celostne in individualne pristope, ki dajejo poudarek izboljšanju telesne strukture, funkcije ter hkrati spodbujajo njihovo samostojno participacijo. Metode: Študija primera je vključevala devetletnega dečka s cerebralno paralizo. Med nevrofizioterapevtskimi obravnavami z Vojto terapijo smo z eksperimentalno metodo zbirali kvantitativne podatke srčnega utripa in variabilnosti srčne frekvence. S programskimi orodji smo izračunali povprečne vrednosti preučevanih parametrov v obdobjih pred, med in po intervencijah. Za ugotavljanje statistično značilnih razlik med meritvami smo uporabili analizo variance za ponovljene meritve. Rezultati: Analiza je pokazala statistično značilno razliko (p = 0,002) v vrednostih srčnega utripa med obdobjema pred nevrofizioterapevtsko intervencijo in med njo, natančneje med izvajanjem stimulacije Vojta terapije. V povprečju se je vrednost srčnega utripa med izvajanjem takšne stimulacije znižala za 3,06 udarca/minuto. Hkrati smo ugotovili statistično značilno razliko (p = 0,008) tudi v vrednostih variabilnosti srčne frekvence. V povprečju se je ta v obdobju med stimulacijo zvišala za 25,54 milisekunde. Razprava: Z raziskovalnim delom smo glede na naš individualni primer dečka s cerebralno paralizo ugotovili, da je izvajanje dotičnih nevrofizioterapevtskih tehnik v kombinaciji z Vojta terapijo imelo takojšen fiziološki učinek. Znižanje vrednosti srčnega utripa in sočasno zvišanje variabilnosti srčne frekvence med stimulacijo nakazujeta ugoden vpliv terapije na avtonomni živčni sistem dečka preiskovanca.

Abstract:Introduction: Cerebral palsy is the most common physical disability in childhood, resulting from brain injury or abnormal brain development. Early diagnosis is crucial, since it allows the child to be included in appropriate therapeutic interventions as soon as possible. Adequate physiotherapeutic treatments for children with cerebral palsy involve effective, holistic, and individualized approaches that emphasize improving body structure and function while simultaneously promoting independent participation. Methods: This case study involved a nine-year-old boy with cerebral palsy. During neurophysiotherapeutic treatments using Vojta therapy, quantitative data on heart rate and heart rate variability were collected using an experimental method. Software tools were used to calculate the mean values of the studied parameters during the periods before, during, and after the interventions. To determine statistically significant differences between measurements, repeated measures analysis of variance (ANOVA) was used. Results: The analysis showed a statistically significant difference (p = 0.002) in heart rate values between the period before the neurophysiotherapeutic intervention and during it, specifically during the application of Vojta therapy stimulation. On average, the heart rate decreased by 3.06 beats/minute during the stimulation. Concurrently, a statistically significant difference (p = 0.008) was also found in heart rate variability values. On average, this increased by 25.54 milliseconds during the stimulation period. Discussion: Based on our individual case of a boy with cerebral palsy, the research indicates that the implementation of specific neurophysiotherapeutic techniques in combination with Vojta therapy had an immediate physiological effect. The decrease in heart rate and the simultaneous increase in heart rate variability during stimulation suggest a favorable impact of the therapy on the subject's autonomic nervous system.
