Title:Učinki progresivne vadbe na tendinopatijo ahilove tetive - pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Diplomsko delo obravnava tendinopatijo Ahilove tetive z vidika sodobnega fizioterapevtskega pristopa s poudarkom na progresivnem obremenjevanju kot ključnem terapevtskem pristopu. Namen raziskave je bil ugotoviti učinkovitost progresivne vadbe ter opredeliti najpogosteje uporabljene vadbene protokole pri rehabilitaciji te patologije. Raziskava temelji na kvalitativni analizi znanstvene literature, ki je bila objavljena v zadnjih desetih letih in pridobljena iz podatkovnih baz PEDro, PubMed, ProQuest, ScienceDirect in Cochrane. V analizo so bile vključene študije, ki so obravnavale vpliv progresivne vadbe na bolečino, funkcionalno zmogljivost in strukturne spremembe Ahilove tetive pri osebah z diagnosticirano tendinopatijo Ahilove tetive. Rezultati kažejo, da vadbeni protokoli, ki temeljijo na progresivnem obremenjevanju, predstavljajo temelj sodobne rehabilitacije tendinopatije Ahilove tetive. Ugotovljeno je bilo, da takšen pristop učinkovito zmanjšuje bolečino, izboljšuje funkcionalno zmogljivost in povečuje toleranco tetive na mehanske obremenitve. V analizo je bilo vključenih več učinkovitih pristopov k rehabilitaciji, kot so ekscentrična, izometrična in kombinirana vadba. Zaradi raznolikosti pristopov ni mogoče opredeliti ene optimalne oblike vadbe. Skupna značilnost vseh pristopov je bila vključitev strukturiranega in postopno stopnjevanega obremenitvenega programa, kar potrjuje progresivno obremenjevanje kot temelj sodobne fizioterapevtske obravnave tendinopatije Ahilove tetive. Kljub pozitivnim ugotovitvam raziskave je razvidno, da je zaradi heterogenosti protokolov potrebno nadaljnje raziskovanje za opredelitev optimalnih parametrov vadbene obremenitve.

Abstract:This thesis examines Achilles tendinopathy from the perspective of modern physiotherapy, with a focus on progressive loading as a key therapeutic approach. The aim of the study was to evaluate the effectiveness of progressive exercise and to identify the most commonly used rehabilitation protocols in the treatment of this condition. The study is based on a qualitative analysis of scientific literature published within the last ten years and obtained from the PEDro, PubMed, ProQuest, ScienceDirect, and Cochrane databases. The analysis included studies investigating the effects of progressive exercise on pain, functional performance, and structural changes of the Achilles tendon in individuals diagnosed with Achilles tendinopathy. The results indicate that exercise protocols based on progressive loading represent the foundation of contemporary rehabilitation for Achilles tendinopathy. It was found that this approach effectively reduces pain, improves functional capacity, and increases the tendon’s tolerance to mechanical loading. Several effective rehabilitation methods were identified, including eccentric, isometric, and combined exercise approaches. Due to the variability of these approaches, it is not possible to define a single optimal form of exercise. However, a common feature across all approaches was the inclusion of a structured and progressively increased loading program, confirming progressive loading as a fundamental principle of modern physiotherapy management of Achilles tendinopathy. Despite the positive findings, the analysis also highlights limitations related to the heterogeneity of rehabilitation protocols, indicating the need for further research to define optimal loading parameters.
