Title:Učinkovitost udarnih valov pri zdravljenju tendinopatije ahilove tetive v primerjavi z običajno konzervativno fizioterapijo - pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Teoretična izhodišča: Ahilova tetiva je največja in najmočnejša tetiva v človeškem telesu, zato je pogosto preobremenjena. Posledično je Ahilova tendinopatija ena najpogostejših poškodb tetiv spodnjih okončin in pomemben klinični ter rehabilitacijski problem. Najpogostejša konzervativna oblika zdravljenje Ahilove tendinopatije je terapevtska, predvsem ekscentrična vadba, kot potencialno učinkoviti dodatek običajnemu konzervativnemu zdravljenju pa se uporablja tudi terapija z udarnimi valovi. Namen: Pregledati znanstveno in strokovno literaturo o tendinopatiji Ahilove tetive in o fizioterapevtskih metodah zdravljenja s poudarkom na terapiji z udarnimi valovi. Metoda: Izveden je bil pregled literature. Članke smo iskali v podatkovnih bazah PubMed, PEDro, ProQuest in SpringerLink. Uporabljeni so bili vključitveni in izključitveni kriteriji. Postopek izbora je bil prikazan s PRISMA diagramom. Randomizirane raziskave so bile ocenjene z lestvico PEDro. Rezultati: V analizo je bilo vključenih 8 študij s 1154 preiskovanci. Uporabljene intervencije so bile: terapija z udarnimi valovi, ekscentrične vaje, raztezanje, laserska terapija, terapija z ultrazvokom, obravnava mehkih tkiv, pristop vibracije celega telesa in spremljanje brez aktivnega zdravljenja. Razprava: Večina raziskav je pokazala statistično pomembno zmanjšanje bolečine in izboljšanje funkcije v vseh skupinah. V nekaterih raziskavah je terapija z udarnimi valovi pokazala dodatno izboljšanje, predvsem v kombinaciji z ekscentrično vadbo ali pri daljšem spremljanju, vendar ne v vseh primerih. Sklep ali zaključek: Ekscentrična vadba ostaja temelj zdravljenja Ahilove tendinopatije. Terapija z udarnimi valovi je lahko koristen dodatek, vendar ni vedno učinkovitejša od običajne konzervativne fizioterapije. Zaradi omejitev obravnavanih raziskav ni mogoče podati enotnih priporočil.

Abstract:Theoretical Background: The Achilles tendon is the largest and strongest tendon in the human body and is frequently subjected to excessive strain. Consequently, Achilles tendinopathy is one of the most common tendon injuries of the lower extremities and a significant clinical and rehabilitation challenge. The most common conservative form of treatment is therapeutic exercise, particularly eccentric exercise. Shockwave therapy is also used as a potentially effective adjunct to standard conservative treatment. Objective: To review the scientific and professional literature on Achilles tendinopathy and physiotherapy treatment methods, with an emphasis on shockwave therapy. Method: A literature review was conducted. Articles were searched for in the PubMed, PEDro, ProQuest and SpringerLink databases. Inclusion and exclusion criteria were applied. The selection process was illustrated using a PRISMA diagram. Randomized studies were assessed using the PEDro scale. Results: Eight studies involving 1,154 participants were included in the analysis. The interventions used were: shockwave therapy, eccentric exercises, stretching, laser therapy, ultrasound therapy, soft tissue manipulation, whole-body vibration and active monitoring without active treatment. Discussion: Most studies showed a statistically significant reduction in pain and improvement in function across all groups. In some studies, shockwave therapy demonstrated additional improvement, particularly when combined with eccentric exercise or during longer follow-up periods, though not in all cases. Conclusion: Eccentric exercise remains the cornerstone of treatment. Shockwave therapy may be a useful adjunct, but it is not always more effective than standard conservative physical therapy. Due to limitations across studies, it is not possible to provide uniform recommendations.
