Title:Pomen fizioterapije v prenatalnem obdobju za potek in izid poroda : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Teoretična izhodišča: V nosečnosti se zaradi hormonskih in biomehanskih sprememb pojavijo telesne spremembe, ki lahko vodijo v disfunkcije. Te lahko otežijo porod in ostanejo prisotne v poporodnem obdobju. S fizioterapevtsko obravnavo, komprehenzivnimi programi za preprečevanje nastanka in omilitev posledic fizioloških sprememb je potencialno mogoče vplivati na potek in izid poroda ter nosečnosti za otroka in mati. Metoda: V diplomskem delu raziskujemo povezavo med fizioterapevtsko obravnavo, edukacijo pacientke in obporodno fizioterapevtsko intervenco ter potekom in izidom poroda s pregledom literature in kvalitativnim raziskovalnim pristopom. Pregled je izveden z iskanji ključnih besed v izbranih podatkovnih bazah, pridobljene vsebine pa so vključene v diplomsko delo na podlagi vključitvenih in izključitvenih kriterijev ter ocene ustreznosti na podlagi podrobnejše analize posameznih raziskav. Rezultati: Vključenih je 11 raziskav. 5 raziskav je usmerjenih v obdobje tik pred porodom in med njim, 6 raziskav pa obravnava obdobje med nosečnostjo. Vse raziskave obravnavajo posledice fizioterapevtske obravnave na potek in izid poroda ter poporodno stanje porodnice. Razprava: Pri pregledu smo ugotovili, da za urinsko inkontinenco in bolečino v hrbtu ter medenici, kot tudi za potek in izid poroda, obstajajo statistično značilne povezave med fizioterapevtsko obravnavo in pojavnostjo disfunkcij ter potekom poroda. Za distazo rektusov raziskava ni ugotovila statistično značilnih povezav. Vsem raziskavam je skupno, da so ugotovljene razlike med kontrolnimi in intervencijskimi skupinami, torej je mogoče zaključiti, da fizioterapija ima vpliv, kolikšen ta je, pa bo mogoče določiti na podlagi nadaljnjega raziskovanja in časovnega odmika.

Abstract:Theoretical background: Due to hormone and biomechanical changes during pregnancy, changes occur in the body of the pregnant woman. These changes can lead to dysfunctions which can make labour more difficult and remain present through the post-partum period. With the assistance of physiotherapy, comprehensive programmes designed to prevent the incidence and soften the intensity of the consequence of physiological changes it is potentially possible to influence the course and outcome of labour. Method: This thesis researches the connection between prenatal physiotherapy, patient education and intervention during labour and the course and outcome of labour through literature review and a qualitative research approach. This review was conducted by searching through chosen databases with keywords, the results filtered through inclusion and exclusion criteria, and the end results once more analysed for inclusion. Results: 11 articles were included, 5 of them focused on the period immediately preceeding labour and during labour, the other 6 focused on the pregnancy period as a whole. All articles research the consequence of prenatal physiotherapy on the course and outcome of labour. Discussion: During the review we discovered a strong statistically significant influence of physiotherapy on the incidence of urine incontinence, back and pelvic pain, as well as labour pain and duration. We did not discover a statistically significant influence of physiotherapy on diastasis recti, however, in all reviews, there was a difference between the control and intervention group, so it is possible to conclude that physiotherapy does have an influence – the amount will be possible to determine based on future research and time.
