Title:Regionalni model socialne infrastrukture za izvajanje integrirane dolgotrajne oskrbe v skupnosti : doktorska disertacija študijskega programa tretje bolonjske stopnje Socialna gerontologija

Abstract:Doktorska disertacija obravnava razvoj regionalnega modela socialne infrastrukture za izvajanje integrirane dolgotrajne oskrbe v skupnosti v Sloveniji, katere poudarek je na razpršenosti poselitve prebivalstva ter vplivu logistike na stroške izvajanja dolgotrajne oskrbe. Z raziskavo se je preverjala razlika v preferencah glede izbire storitev in tipa bivanja v dolgotrajni oskrbi. Analizirani so bili stroški bivanja in oskrbe v domačem okolju, domu starejših občanov in varovanih stanovanjih. Uporabljena je bila mešana metoda raziskovanja: pregled literature, izvedba intervjujev z uporabniki dolgotrajne oskrbe na domu, v oskrbovanih stanovanjih in v domu starejših občanov, opravljena je bila kvalitativna analiza intervjujev, izvedena je bila anketa na vzorcu 1005 oseb z 475 ustreznimi odgovori, opravljena je bila kvantitativna analiza anketnega vprašalnika, ter stroškovni izračuni in primerjave med različnimi tipi bivanja. Raziskava je pokazala, da so statistično značilne razlike v preferenci po starostnih skupinah, ni pa pokazala statistično značilnih razlik v preferencah po spolu in statusu. Večina anketiranih v primeru odvisnosti od pomoči druge osebe preferira selitev od doma do največ 10 km. Stroškovne analize izvajanja dolgotrajne oskrbe kažejo, da bi lahko prihranke, ki izhajajo iz poti zaposlenih, namenili delnemu financiranju najemnine specializiranih stanovanjskih enot. Disertacija s predlaganim modelom kaže na stroškovno vzdržne rešitve porazdelitve tipov bivanja v dolgotrajni oskrbi z upoštevanjem potreb in želja starejših odraslih. Rezultati so pomembni za ustvarjalce politik dolgotrajne oskrbe, saj je razumevanje vpliva prostora na stroške izvajanje dolgotrajne oskrbe in preference glede tipa socialne infrastrukture, v katerem posameznik prejema socialno infrastrukturo, pomembna za dolgoročno planiranje in vzdržnost sistema dolgotrajne oskrbe.

Abstract:The doctoral dissertation addresses the development of a regional model of social infrastructure for the provision of integrated long-term care in the community in Slovenia, with a particular focus on dispersed settlement patterns and the impact of logistics on the costs of long-term care delivery. The study examined differences in preferences for types of services and living arrangements within long-term care. The analysis included costs of living and care in the home environment, residential care homes for older adults, and assisted (sheltered) housing. A mixed-methods research design was applied, comprising a systematic literature review, interviews with long-term care users receiving care at home, in assisted housing, and in residential care homes, followed by qualitative analysis of interview data. In addition, a survey was conducted on a sample of 1,005 individuals, yielding 475 valid responses, which were subjected to quantitative analysis. Furthermore, cost calculations and comparisons between different living arrangements were performed. The results demonstrate statistically significant differences in preferences across age groups, while no statistically significant differences were identified with respect to gender or status. The majority of respondents prefer to relocate no more than 10 km from their original home in the event of dependence on assistance. Cost analyses of long-term care provision indicate that savings arising from reduced travel distances for care staff could be partially redirected towards financing the rent of specialised housing units. The dissertation and the proposed model present cost-efficient and sustainable solutions for the spatial distribution of different long-term care living arrangements, taking into account the needs and preferences of older adults. The findings are highly relevant for long-term care policymakers, as understanding the spatial dimension of care costs and user preferences regarding types of social infrastructure is essential for long-term planning and the sustainability of the long-term care system.
