Title:FIZIOTERAPEVTSKA OBRAVNAVA PO OPERACIJI SPINALNE STENOZE LEDVENE HRBTENICE

Abstract:Uvod in namen: Ledvena spinalna stenoza (LSS) se najpogosteje pojavlja pri starejši populaciji in se kaže z bolečino ali spremenjeno senzibiliteto v predelu ledvene hrbtenice in nogah. Prva izbira je konzervativno zdravljenje, če to ne pomaga, se posledično odloči za operativni poseg oziroma dekompresijo. Po operativnem posegu je rehabilitacija usmerjena v strukturiran kinezioterapevtski program, ki vključuje napredujoč skupek vaj za splošno mišično moč, stabilizacijo jedra in izboljšanje ravnotežja. Učinkovitost številnih fizioterapevtskih metod in tehnik po operaciji ni povsem znana. Ugotovili smo, da se intenzivnost bolečine hitreje zmanjša pri bolnikih, ki so deležni strukturiranega programa rehabilitacije. Metode: Diplomska naloga temelji na sistematičnem pregledu literature, pridobljene s pomočjo elektronskih baz podatkov PubMed, Google Scholar in PEDro. Rezultati: V analizo smo vključili sedem raziskav. Ugotovili smo zanesljive dokaze o učinkovitosti kinezioterapije pri zdravljenju LSS po dekompresiji. Visokokakovostnih študij, ki bi potrdile dolgoročno učinkovitost, ni. Uporabnost: V sklopu fizioterapevtske obravnave pacientov po operaciji LSS priporočamo izvajanje kinezioterapije, ki vključuje mobilizacijske vaje z raztezanjem in progresivno mišično močjo za korekcijo drže, stabilizacijo ledvenega dela, aerobne vaje in športno specifično vadbo. Omejitve: Večina raziskav temelji na majhnem vzorcu preiskovancev in kratkem trajanju raziskave, zato zanesljivega programa rehabilitacije ni mogoče oblikovati.

Title:Physiotherapeutic treatment after lumbar spinal stenosis surgery
Abstract:Introduction and purpose: Introduction and Purpose: Lumbar spinal stenosis (LSS) occurs most commonly in the elderly population and is characterised by pain or altered sensation in the lumbar spine and legs. Conservative treatment is the first choice, and if this does not help, surgical intervention or decompression is chosen. After surgery, rehabilitation is focused on a structured kinesiotherapy programme, including a progressive set of exercises for general muscle strength, core stabilisation and balance improvement. The effectiveness of many physiotherapy methods and techniques after surgery is not fully known. We have found that pain intensity decreases more rapidly in patients who receive a structured rehabilitation programme. Methods: The thesis is based on a systematic review of the literature obtained from the electronic databases PubMed, Google Scholar and PEDro. Results: 7 studies were included in the analysis. We found reliable evidence of the effectiveness of kinesiotherapy in the treatment of LSS after spinal decompression. There are no high-quality studies confirming long-term efficacy. Usability: As part of the physiotherapy treatment of patients after LSS surgery, we recommend kinesiotherapy, which includes mobilisation exercises with stretching and progressive muscle strength to correct posture, stabilise the lumbar spine, aerobic exercises and sports-specific exercise. Limitations: Most studies are based on a small sample size and short study duration, so a reliable rehabilitation programme cannot be designed.
