Title:Uvajanje kazalnika higiene rok v prizadevanjih za varnost in kakovost oskrbe starejših v domovih za starejše

Abstract:Izhodišča: Preprečevanje okužb, povezanih z zdravstvom, je eden od pomembnih kazalnikov kakovosti dela zdravstvenih delavcev ter zdravstvenih in socialnovarstvenih zavodov. Zdravstvena oskrba v socialnovarstvenih zavodih je povezana s specifičnim tveganjem nastanka okužb, povezanih z zdravstvom. Zaposleni so dolžni izvajati dejavnosti ter ukrepe, ki vodijo k preprečevanju nastanka okužb, povezanih z zdravstvom, saj s tem ohranjajo kakovostno in varno zdravstveno in socialnovarstveno oskrbo stanovalcev v domovih za starejše. Metode: Magistrsko delo je temeljilo na kvalitativnem in kvantitativnem raziskovalnem pristopu, v začetnem delu je bil opravljen pregled literature. Literaturo smo poiskali s pomočjo iskalnika COBBIS, pod ključnimi besedami »higiena rok«, »pet trenutkov za higieno rok«, »izobraževanje«, »usposabljanje«, »opazovalec higiene rok«, »strokovna navodila za higieno rok«, »potreba po razkuževanju«, »kakovost zdravstvene obravnave«, »varnostni odkloni«. Poslužili smo se tudi elektronskih virov, in sicer podatkovne baze CHINAL ter bibliografske baze podatkov zdravstvene nege, Medline in Pub Med. Prav tako smo se poslužili tudi spletne strani Svetovne zdravstvene organizacije in Ministrstva za zdravje RS, v namen dostopa do ustreznih zakonov in pravnih aktov. V drugem delu smo za opazovanje uporabili obrazec za opazovanje higiene rok. Obrazec je izdelan po metodologiji World Health Organization in se v bolnišnicah že uporablja za opazovanje higiene rok. Rezultati: V skupnem izračunu vseh opazovanih profesionalnih skupin smo pridobili podatek, da je bilo 91,6 % zaposlenih pri svojem delu doslednih pri higieni rok. Delež diplomiranih medicinskih sester, fizioterapevtov in delovnih terapevtov pri doslednosti higiene rok je bil 100 %, zdravstvenih tehnikov 93,9 %, bolničarjev negovalcev 93,2 %, socialnih oskrbovalk, strežnic, izvajalcev javnih del pa 85,3 %. Med opazovanimi profesionalnimi kategorijami sta bili tudi socialni delavki, ki sta dosegli skupno 87,5 % doslednost higiene rok. Razprava: Podatek raziskave v Domu starejših občanov Črnomelj po navedbah Svetovne zdravstvene organizacije nakazuje zelo dober rezultat, saj se zaposleni v zadostni meri zavedajo pomena higiene rok, dosegajo več kot 70 % doslednost pri higieni rok in s tem stremijo k preprečevanju prenosa okužb med stanovalci in med zaposlenimi. Kljub dobremu rezultatu pa se moramo zavedati, da še vedno obstajajo izboljšave na področju higiene rok, zato moramo vse zaposlene v zavodu kontinuirano izobraževati in redno izvajati opazovanja higiene rok.

Title:Intoducting an indicator of hand hxgiene in an effort to maintain the safety and quality of car for the eldery in old people´s homes
Abstract:Introduction: Preventing healthcare-associated infections is a crucial indicator of the quality of work in healthcare professionals and healthcare and social welfare institutions. Healthcare in social welfare institutions is associated with a specific risk of healthcare-associated infections. Employees are obligated to implement activities and measures that lead to the prevention of healthcare-associated infections, as this ensures high-quality and safe healthcare and social care for residents in nursing homes. Methods: This master's thesis employed both qualitative and quantitative research approaches. In the initial phase, a literature review was conducted. Literature was retrieved using the COBBIS search engine with keywords such as "hand hygiene," "five moments for hand hygiene," "education," "training," "hand hygiene observer," "professional hand hygiene guidelines," "need for disinfection," "quality of healthcare," and "safety deviations." Electronic sources, including the CHINAL database and bibliographic databases related to nursing care, Medline, and Pub Med, were also utilized. Additionally, information from the World Health Organization and the Ministry of Health of the Republic of Slovenia was accessed to refer to relevant laws and legal acts. In the second part, hand hygiene compliance was observed using a form based on the methodology of the World Health Organization, which is already in use in hospitals for hand hygiene observation. Results: In the overall assessment of all observed professional groups, it was found that employees exhibited a hand hygiene compliance rate of 91.6%. Registered nurses, physiotherapists, and occupational therapists achieved 100% compliance in hand hygiene. Healthcare technicians achieved 93.9% compliance, nursing assistants reached 93.2%, while social care workers, attendants, and public workers achieved an 85.3% hand hygiene compliance rate. Among the observed professional categories, social workers achieved a combined 87.5% hand hygiene compliance. Discussion: The research findings at the Nursing Home for Elderly Citizens Črnomelj, in line with the World Health Organization's recommendations, indicate a very good result. Employees are sufficiently aware of the importance of hand hygiene, achieving more than 70% compliance, thereby striving to prevent the transmission of infections among residents and staff. Despite the good result, there is still room for improvement in hand hygiene. Therefore, it is essential to continuously educate and regularly observe hand hygiene compliance among all employees in the institution.
