Title:TERAPEVTSKI PRISTOPI ZA OBVLADOVANJE POSLEDIC SPOLNE ZLORABE OTROK: PREGLED IN PRIMERJAVA TF-CBT IN GB-CBT

Abstract:Teoretična izhodišča: Spolna zloraba otrok predstavlja perečo problematiko in kompleksno travmatsko izkušnjo, ki lahko povzroči različne dolgoročne duševne posledice v življenju žrtve. Nova spoznanja na duševnem področju in integracija psihoterapevtskih metod so doprinesla k razvoju različnih oblik kognitivno-vedenjske terapije, med katerimi so tudi pristopi, ki so se izkazali za uspešne pri obvladovanju različnih posledic spolne zlorabe. Namen je raziskati osnovne koncepte in teoretične okvire terapevtskih pristopov TF-CBT in GB-CBT ter njihovo učinkovitost pri obravnavi spolne zlorabe otrok. Metoda: Uporabili smo kritični pregled strokovne in znanstvene literature s pomočjo podatkovnih baz Google Scholar, Science Direct, Sage Journals ter PubMed. Vključitveni kriteriji so: besedilo vsebuje relevantne informacije o obravnavani temi; učinkovitost pristopov TF-CBT in GB-CBT pri obravnavi spolno zlorabljenih otrok; primerjava pristopov TF-CBT in GB-CBT pri obravnavi spolno zlorabljenih otrok; oblika obravnave kognitivno-vedenjskega pristopa je jasno opredeljena; polna dostopnost do besedila. V pregledu smo upoštevali PRISMA-standarde. Rezultati: Od začetnih 170.278 zadetkov smo v končno vsebinsko analizo po pregledu celotnega besedila (133) vključili 32 virov. Po hierarhiji dokazov smo imeli različne ravni kakovosti dokazov: sistematični pregled nerandomiziranih kliničnih študij in metaanalize (1), posamezne randomizirane klinične študije (2), posamezne nerandomizirane klinične študije (8), posamezne korelacijske/opazovalne študije (2), sistematični pregled opisnih/kvalitativnih študij (4), posamične opisne/kvalitativne študije (2), mnenja avtorjev/ekspertnih komisij (7). Ugotovitve: Ugotovitve kažejo, da sta tako TF-CBT kot GB-CBT učinkovita pri zmanjševanju simptomov posttravmatske stresne motnje (PTSM), depresije, anksioznosti in drugih psiholoških težav pri otrocih, ki so bili žrtve spolne zlorabe. Prilagajanje terapevtskega procesa razvojni stopnji otroka je ključnega pomena, še posebej pri predšolskih otrocih. V zaključku podamo tudi predloge za nadaljnje raziskave.

Title:THERAPEUTIC APPROACHES FOR MANAGING THE CONSEQUENCES OF CHILD SEXUAL ABUSE: OVRERVIEW AND COMPARISON OF TF-CBT AND GB-CBT
Abstract:Theoretical Background: Child sexual abuse represents a pressing issue and a complex traumatic experience that can result in various long-term psychological consequences in the victim's life. New insights in the field of psychology and the integration of psychotherapeutic methods have contributed to the development of various forms of cognitive-behavioral therapy, among which are approaches that have proven successful in addressing the diverse consequences of child sexual abuse. The aim is to explore the basic concepts and theoretical frameworks of TF-CBT and GB-CBT therapeutic approaches and their effectiveness in addressing child sexual abuse. Method: We conducted a critical review of professional and scientific literature using databases such as Google Scholar, Science Direct, Sage Journals, and PubMed. Inclusion criteria were as follows: the text contains relevant information on the topic under consideration; effectiveness of TF-CBT and GB-CBT approaches in addressing sexually abused children; comparison of TF-CBT and GB-CBT approaches in addressing sexually abused children; the form of cognitive-behavioral approach is clearly defined; full accessibility to the text. PRISMA standards were adhered to in the review. Results: Out of the initial 170,278 hits, we included 32 sources in the final content analysis after reviewing the entire text (133). According to the hierarchy of evidence, we had various levels of evidence quality: systematic review of non-randomized clinical studies and meta-analyses (1), individual randomized clinical studies (2), individual non-randomized clinical studies (8), individual correlational/observational studies (2), systematic review of descriptive/qualitative studies (4), individual descriptive/qualitative studies (2), expert opinions/commissioned reviews (7). Findings: The findings indicate that both TF-CBT and GB-CBT are effective in reducing symptoms of post-traumatic stress disorder (PTSD), depression, anxiety, and other psychological problems in children who have been victims of sexual abuse. Adapting the therapeutic process to the developmental stage of the child is crucial, especially for preschool children. In conclusion, we provide suggestions for further research.
