Title:Učinkovitost fizioterapevtske obravnave pri pacientih z adhezivnim kapsulitisom rame

Abstract:Adhezivni kapsulitis oziroma sindrom zamrznjene rame je pogosta samoomejujoča patologija ramenskega sklepa, za katero so značilne bolečina, togost in progresivna izguba tako pasivnega kot aktivnega obsega gibljivosti v ramenskem sklepu. Kljub novejšim raziskavam, s katerimi se je izboljšalo razumevanje zamrznjene rame, je natančen patofiziološki proces še vedno slabo definiran, večina raziskav pa je enotnih, da gre za kapsularno vnetje s kasnejšo fibrozo. Dejavniki tveganja so poškodba rotatorne manšete, diabetes, bolezni ščitnice, miokardni infarkt, možganska kap in različne avtoimunske bolezni. V večini primerov je diagnoza klinična, slikovna diagnostika pa se uporablja predvsem za izločanje drugih stanj. Zaradi dokaj slabega razumevanja patološkega stanja in pomanjkanja raziskav pogosto predstavlja izziv pri postavljanju diagnoze, kot tudi pri samem zdravljenju. Bolezen je pogosto samoomejujoča, vendar lahko traja več let, v nekaterih primerih pa simptomi nikoli ne prenehajo v celoti. Z namenom izboljšanja stanja je najpogosteje predpisana oblika zdravljenja fizioterapija, katere cilj je zmanjšati bolečino in prizadetost, izboljšati gibljivost ramenskega sklepa ter izboljšati kakovost življenja. V diplomskem delu so podrobno predstavljene anatomija ramenskega sklepa, patologija adhezivnega kapsulitisa, njegova klinična slika, diagnoza ter fizioterapevtske metode, tehnike in njihova učinkovitost pri zdravljenju. Zaradi slabega razumevanja patološkega stanja priporočamo izvedbo nadaljnjih obsežnih raziskav na področju patofiziologije, etiologije, diagnoze in zdravljenja adhezivnega kapsulitisa. Analiza najnovešje znanstvene in strokovne literature je pokazala pomembne kratkoročne in dolgoročne učinke kombiniranja različnih tehnik manualne terapije, kinezioterapije in fizikalnih modalitet na zmanjšanje bolečine, prizadetosti ter povečanje aktivnega in pasivnega obsega gibljivosti glenohumeralnega sklepa pri bolnikih z adhezivnim kapsulitisom rame. Če si prizadevamo v prihodnosti določiti smernice oziroma najučinkovitejše protokole fizioterapevtske obravnave pri zdravljenju zamrznjene rame, bo potrebno izvesti dodatne visokokakovostne študije, osredotočene na učinkovitost posamezne fizioterapevtske metode oziroma tehnike.

Title:Effectiveness of physical therapy in patients with adhesive capsulitis of the shoulder
Abstract:Adhesive capsulitis or frozen shoulder syndrome, is a common self-limiting pathology of the shoulder joint characterized by pain, stiffness and progressive loss of active and passive range of motion. Despite recent research that has improved the understanding of frozen shoulder, the exact pathophysiological process remains poorly defined, with most studies agreeing that it involves capsular inflammation followed by fibrosis. Risk factors include rotator cuff injuries, diabetes, thyroid diseases, myocardial infarction, stroke, and various autoimmune diseases. In most cases, the diagnosis is clinical, with imaging diagnostics primarily used to rule out other conditions. Due to the relatively poor understanding of the pathological state and the lack of research, diagnosing and treating this condition often presents a challenge. The disease is often self-limiting but can last for several years, and in some cases, symptoms may never fully resolve. The most commonly prescribed form of treatment is physical therapy, aiming to reduce pain and disability, improve the range of motion in the shoulder joint, and enhance the quality of life. The thesis provides a detailed presentation of the shoulder joint's anatomy, the pathology of adhesive capsulitis, its clinical picture, diagnosis, and physiotherapeutic methods, techniques, and their effectiveness in treatment. Due to the poor understanding of the pathological state, further extensive research in the fields of pathophysiology, etiology, diagnosis, and treatment of adhesive capsulitis is recommended. Analysis of the latest scientific and professional literature has shown significant short-term and long-term effects of combining various manual therapy techniques, kinesiotherapy, and physical modalities on reducing pain and disability and increasing the active and passive range of motion in the glenohumeral joint in patients with adhesive capsulitis of the shoulder. To determine guidelines or the most effective physical therapy protocols for treating frozen shoulder in the future, additional high-quality research focused on the effectiveness of individual physiotherapeutic methods or techniques will be necessary.
