Title:VLOGA PATRONAŽNE MEDICINSKE SESTRE PRI PREPOZNAVANJU POPORODNE DEPRESIJE

Abstract:Izhodišča: Poporodna depresija je pri otročnicah najpogostejša duševna motnja. Dejavniki tveganja za nastanek so predhodne duševne motnje, pomanjkanje podpore s stani partnerja ali družine otročnice, slabe socialno-ekonomske razmere, nasilje nad otročnico. Simptomi poporodne depresije so različni, od prekomerne skrbi za novorojenčka do popolnega zavračanja novorojenčka, apatije, jokavosti, občutkov krivde. Zdravljenje je farmakološko ali nefarmakološko. Otročnicam v Sloveniji pripadata dva obiska patronažne medicinske sestre. Namen raziskave je ugotoviti vlogo patronažne medicinske sestre pri prepoznavanju poporodne depresije. Metode: Raziskava je temeljila na kvalitativni in deskriptivni metodi dela. Za potrebe empiričnega dela so bili zbrani, analizirani ter sintetizirani primarni in sekundarni viri. Zbrani podatki za analizo so bili pridobljeni s tehniko intervjuvanja. Sekundarni so bili pridobljeni s pregledom strokovne domače in tuje literature ter spletnih baz. Rezultati: Intervjuvanke se na svojih terenih srečujejo s poporodno depresijo, vendar ne pogosto. Večina jih meni, da pojav poporodne depresije ni pogost, dve od njih menita, da pojav narašča. V letu 2023 so štiri intervjuvanke obravnavale po en primer poporodne depresije, ena dva primera in ena nobenega. Vse intervjuvanke poznajo in uporabljajo Edinburški vprašalnik poporodne depresije za prepoznavanje poporodne depresije. Kot metodo za prepoznavanje poporodne depresije uporabljajo pogovor z otročnico. Po presoji, da ta potrebuje pomoč, se obrnejo na njenega osebnega zdravnika. Kot način zdravljenja poznajo psihoterapijo in zdravljenje z zdravili. Intervjuvane patronažne medicinske sestre dobro poznajo znake poporodne depresije in dejavnike tveganja za njen nastanek. Razprava: Ugotovljeno je bilo, da se vzorec intervjuvanih patronažnih medicinskih sester ne srečuje pogosto s poporodno depresijo, vendar bi bilo v smeri pogostosti srečanja patronažnih medicinskih sester s poporodno depresijo potrebno izvesti še dodatne raziskave. Glavni način prepoznavanja poporodne depresije s strani patronažne medicinske sestre je pogovor z otročnico. Pri sumu na poporodno depresijo je kontaktiranje osebnega zdravnika otročnice in ostalih strokovnjakov glavni pristop patronažne medicinske sestre. Patronažne medicinske sestre dobro poznajo znake poporodne depresije, jo znajo prepoznati, dobro poznajo dejavnike tveganja in vedo, katere otročnice so bolj ogrožene za nastanek poporodne depresije.

Title:THE ROLE OF FAMILY NURSE IN RECOGNITION OF PASTPARTUM DEPRESSION
Abstract:Baseline: Postpartum depression is the most common mental disorder among mothers. Risk factors for occurrence are previous mental disorders, lack of support from the mother’s partner or family, poor socio-economic conditions, violence against the mother. Symptoms of postpartum depression are different, ranging from excessive worry for the newborn, to complete rejection of the newborn, apathy, crying, feelings of guilt. Treatment is pharmacological or non-pharmacological. Mothers in Slovenia have two visits of a family nurse. The purpose of the research is to determine the role of family nurses in recognizing postpartum depression. Methods: The research was based on a qualitative and descriptive method of work. For the purposes of the empirical part, primary and secondary sources were collected, analyzed and synthesized. The data collected for analysis were obtained using the interviewing technique. Secondary ones were obtained by reviewing professional domestic and foreign literature and online databases. Results: The interviewed ones encounter postpartum depression in their fields, but not often, most of them believe that the occurrence of postpartum depression is not common, two of them believe that the phenomenon is increasing. In 2023 four interviewees dealt with one case of postpartum depression, one dealt with two cases and one dealt with none case. All interviewees know and use the Edinburgh Postpartum Depression Questionnaire to identify postpartum depression. They use talking to the mother as a method of recognizing postpartum depression, and after judging that she needs help, they turn to her doctor. They know psychotherapy and drug treatment as a method of treatment. Interviewed family nurses are aware of the signs and risk factors for postpartum depression. Discussion: It was found that a sample of interviewed family nurses does not often encounter postpartum depression, but further research should be conducted towards the frequency of family nurses meeting postpartum depression. The main way to recognize postpartum depression by a family nurse is talking to the mother. If postpartum depression is suspected, contacting a mother’s personal doctor and other experts is the main approach of a family nurse. Family nurses are well acquainted with postpartum depression, they recognize and act on the signs of postpartum depression, they know risk factors well and know which mothers are more at risk of developing postpartum depression.
