Opis:Izhodišča: Ena od težjih psihiatričnih bolezni je shizofrenija. Za medikamentozno zdravljenje te bolezni se uporabljajo antipsihotiki, ki so bili odkriti v 50-tih letih 20. stoletja. V skupino 1. generacije antipsihotikov med drugim sodi haloperidol, ki se poleg ostalih indikacij uporablja za zdravljenje shizofrenije. Deluje na principu antagonizma dopaminskih receptorjev D2. Kompetenca medicinske sestre je varna aplikacija zdravil, zato je bil namen raziskave opredeliti njeno vlogo pri zdravljenju shizofrenije s haloperidolom. Metoda: Raziskava je temeljila na kvalitativnem raziskovalnem pristopu in deskriptivni metodi dela. Za potrebe empiričnega dela so bili zbrani, analizirani in sintetizirani primarni in sekundarni viri. Primarni podatki za analizo so bili pridobljeni s tehniko intervjuvanja. Intervjuvanih je bilo 6 izvajalcev zdravstvene nege, ki imajo različno dolgo delovno dobo na področju psihiatrije in se srečujejo s pacienti s shizofrenijo. V vzorec so bili zajeti zaposleni v zdravstveni negi na različnih nivojih zdravstvenega varstva. Rezultati: V raziskavi je bilo ugotovljeno, da intervjuvanci zelo dobro poznajo učinke haloperidola in relativno dobro njegovo farmakokinetiko. Z aplikacijo haloperidola pri pacientih s shizofrenijo se srečujejo različno pogosto, odvisno od delovnega mesta. Njihove izkušnje kažejo, da je haloperidol zelo učinkovito zdravilo, ima pa več izraženih neželenih učinkov, ki terjajo skrbno opazovanje oziroma ocenjevanje psihofizičnega stanja pacienta. Intervjuvanci se zavedajo široke vloge medicinske sestre pri zdravljenju shizofrenije s haloperidolom in so jo opredelili na ožji del, ki je neposredno vezan na aplikacijo zdravila ter širši del, ki vključuje sodelovanje z zdravnikom, dokumentiranje, skrb za varnost in pravočasno reakcijo ob zapoznelih neželenih učinkih haloperidola. Zavedajo se nevrotoksičnosti zdravila, vendar dajejo prednost trenutnemu lajšanju pacientovih težav. Etične dileme so v raziskovalnem vzorcu prisotne pri posameznikih predvsem v povezavi s pacientovim odnosom do haloperidola in njegovega odmerka. Rešujejo jih medicinske sestre same ali pa ostajajo nerazrešene. Razprava: Kakovostna zdravstvena oskrba pacientov s shizofrenijo vključuje varno aplikacijo antipsihotikov. V kolikšni meri se pacientom predpisuje haloperidol, je domena zdravnikov, vendar lahko medicinska sestra s svojimi opažanji delno vpliva na odmerek medikamentozne terapije. V okviru zdravstvene nege je pomembno dobro opazovanje pacienta in sodelovanje z zdravnikom, prav tako zdravstvenovzgojno delo s pacientom in svojci. Dobro poznavanje delovanja zdravila je velika prednost in omogoča pravočasen strokovni odziv in predvidevanje možnih zapletov. Z etičnimi dilemami, ki spremljajo aplikacijo haloperdiola, se medicinske sestre spopadajo različno; v primeru moralne stiske bi potrebovale podporo in varno okolje, da bi to svobodno izrazile v timu.

Naslov:THE NURSES ROLE IN THE TREATMENT OF SCHIZOPHRENIA WITH HALOPERIDOLA
Opis:Theory: One of the most severe psychiatric diseases is schizophrenia. Antipsychotics are used for medical treatment of this disease. The discovery of antipsychotics dates to the 1950s. The group of 1st generation of antipsychotics includes haloperidol, which in addition to other indications, is used to treat schizophrenia. It works on the principle of antagonism of D2 dopamine receptors. The nurs's competence is safe application of drugs, so the purpose of the research was to define her role in the treatment of schizophrenia with haloperidol. Method: Research was based on qualitative data and descriptive method of analysis. For the empirical part, primary and secondary sources were gathered, analysed and synthesised. Primary data were obtained using an interview technique. Six nurses who have varying lenghts of experiences in psychiatric nursing of patients with schizophrenia were interviewed. The research included nursing employees at different levels of healthcare system. Results: In the research, it was established that the interviewees know very well the effects of haloperidol and the relatively good pharmacokinetics of the drug, too. It was found that the interviewees encounter the application of haloperidol in patients with schizophrenia with varying frequency, depending on the field they are employed in. The experiences of the interviewees show that haloperidol is a very effective drug, but it has several pronounced side effects that require careful observation or assessment of the patient's psychophysical condition. The interviewees are aware of the broad role of the nurse in the treatment of schizophrenia with haloperidol and define it as a narrower part which is directly related to the application of the drug, and a broader part which includes cooperation with the doctor, documentation, concern for safety and timely reaction in case of delayed side effects of haloperidol. They are aware of the neurotoxicity of the drug, but prefer immediate relief of the patient's problems. Ethical dilemmas are present in individuals in the research sample and are linked to the patients' attitude to haloperidol and to the dose. Ethical dilemmas are solved by nurses themselves or remain unresolved. Discission: Quality medical care for patients with schizophrenia includes safe administration of antipsychotics. To what extent haloperidol is prescribed to patients is the domain of doctors, but the nurse has some influence on the dose of drug therapy with her observations. In the context of nursing care, good observation of the patient and cooperation with the doctor, health education work with the patient and relatives are important. A good knowledge of the drug's action is a great advantage and enables a timely professional response and anticipation of possible complications. If ethical dilemmas occur when applying haloperdiol, nurses face them differently and in case of moral distress, they would need support and a safe environment to dare to express them in the team.
