Title:Kakovost zdravstvene obravnave odraslih na področju duševnega zdravja: izkušnje na primarni ravni

Abstract:Teoretična izhodišča: Duševno zdravje je ključno za posameznika in družbo, duševne motnje pa še vedno predstavljajo velik izziv. Po ocenah WHO bodo do leta 2030 prav te največji javnozdravstveni problem. Slovenija je z Nacionalnim programom duševnega zdravja in Centri za duševno zdravje odraslih naslovila nekatere težave, a stigma, neenak dostop do obravnave in povečane potrebe po zagotavljanju duševnega zdravja ostajajo izzivi tako za strokovnjake, ki delajo na tem področju, kot za odločevalce. Metode: Izvedli smo kvalitativno raziskavo, ki je temeljila na deskriptivni metodi dela, izvedena je bila v okviru ciljnega raziskovalnega projekta CRP2023:V3-2311, Vrzeli med potrebami in celovito, integrirano in kontinuirano zdravstveno obravnavo različnih skupin marginaliziranih odraslih na področju duševnega zdravja. Za instrument zbiranja podatkov smo uporabili fokusno skupino in polstrukturiran intervju. V Fokusni skupini je sodelovalo šest strokovnjakov iz različnih služb, ki se v regiji Posavje vključujejo v skrb za osebe s težavami v duševnem zdravju. Polstrukturirane intervjuje smo opravili s petimi strokovnjaki različnih profilov (psihiatrinja, psihologinja, socialna delavka, delovna terapevtka in diplomirana medicinska sestra), zaposlenimi v Centru za duševno zdravje odraslih Posavje. Intervjuvali smo tudi štiri paciente, obravnavane v istem centru ter politično odločevalko na področju duševnega zdravja. Rezultati: Pacienti ocenjujejo oskrbo v Centru za duševno zdravje odraslih kot kakovostno zaradi individualnega pristopa in dostopnosti. Strokovnjaki izpostavljajo napredek pri skrbi za osebe s težavami v duševnem zdravju, hkrati opozarjajo na pomanjkanje kadrov, infrastrukture in dolgotrajnost rehabilitacije. Omejena dostopnost do podpornih storitev je posledica geografskih dejavnikov in pomanjkanja dnevnih centrov. Stigma ovira iskanje pomoči, preventivni programi so slabo razviti. Posebno ranljive skupine (starejši, brezdomci, socialno ogroženi) imajo omejen dostop do storitev. Razprava: Skupnostna psihiatrična obravnava izboljšuje oskrbo, a ključni izzivi ostajajo: kadrovski primanjkljaj, dolge čakalne dobe, geografske omejitve in stigma. Sodelovanje med psihiatričnimi bolnišnicami in primarno ravnjo oskrbe je pomanjkljivo. Potrebno je okrepiti preventivo, izboljšati sodelovanje med službami znotraj regije, povečati dostopnost in doseči bolj enakomerno razporeditev virov ter izboljšati kulturne kompetence strokovnjakov. Ukrepi so nujni za bolj vključujoč in učinkovit sistem oskrbe.

Title:Adult mental health care quality: experience on the primary care level
Abstract:Theoretical Background: Mental health is essential for individuals and society, yet mental disorders remain a major challenge. WHO estimates they will become the leading public health issue by 2030. Slovenia has addressed some issues through the National Mental Health Program and Community Mental Health Centers, but stigma, unequal access, and increasing demand persist. Methods: We conducted a qualitative study based on a descriptive method of work, carried out in the framework of the targeted research project CRP2023:V3-2311 Gaps between needs and comprehensive, integrated and continuous mental health care for different groups of marginalised adults. Focus group and semi-structured interviews were used as data collection instruments. Six professionals from different services involved in the care of people with mental health problems in the Posavje region participated in the focus group. Semi-structured interviews were conducted with five professionals of different profiles (a psychiatrist, a psychologist, a social worker, an occupational therapist and a registered nurse) working in CDZO Posavje. We also interviewed four patients treated in the same centre and a political decision-maker in the field of mental health. Results: Patients rated Community mental health center’s care as high-quality due to personalized approaches and accessibility. Experts acknowledged improvements but highlighted challenges such as staff shortages, infrastructure gaps, and lengthy rehabilitation. Limited access to support services is linked to geographic barriers and a lack of day centers. Stigma discourages help-seeking, and preventive programs remain underdeveloped. Vulnerable groups (elderly, homeless, socioeconomically disadvantaged) face additional barriers. Discussion: Community-based psychiatric care improves service quality, but key challenges persist such as staff shortages, long waiting times, geographic limitations, and stigma. Collaboration between psychiatric hospitals and primary care is insufficient. Strengthening prevention, improving service coordination, increasing accessibility, and addressing structural barriers are essential for a more inclusive and effective mental health system.
