Title:Vloga diplomirane medicinske sestre pri obravnavi bolnika s septičnim šokom

Abstract:Teoretična izhodišča: Sepsa in septični šok sta urgentni zdravstveni stanji, ki izhajata iz neustreznega sistemskega odziva telesa na okužbo, kar lahko povzroči odpoved organov in smrt. Septični šok je najtežja oblika sepse, pri kateri pride do hude motnje v delovanju organskih sistemov zaradi neustreznega odziva telesa na okužbo. Ključno vlogo pri pravočasnem prepoznavanju in zdravljenju tega stanja ima diplomirana medicinska sestra, saj spremlja vitalne znake, aplicira tekočine, antibiotike in uporablja različne ocenjevalne lestvice ter je pogosto prva, ki opazi zgodnje opozorilne znake in spremembe. Ti lahko vključujejo spremembe v zavesti, hemodinamske in respiratorne nepravilnosti ter laboratorijske kazalnike vnetja. Metode: Raziskava temelji na sistematičnem pregledu literature, izvedenem oktobra 2024, z uporabo podatkovnih baz PubMed, Google Učenjak, Cobiss in ScienceDirect. Uporabljena je bila deskriptivna metoda dela s pregledom strokovne in znanstvene literature v slovenskem in angleškem jeziku, ki je bila povezana s področjem raziskovanja vloge diplomirane medicinske sestre pri obravnavi bolnika s septičnim šokom. Za namen raziskave smo uporabili literaturo znanstveno-raziskovalnega in strokovnega publiciranja, ki je bila prosto dostopna v celotnem obsegu in je bila objavljena v časovnem obdobju od januarja 2014 do oktobra 2024. Rezultati: Na podlagi raziskovalne strategije iskanja s pomočjo ključnih besed smo pri pregledu literature pridobili 916 zadetkov iz različnih znanstvenih baz. V naslednjem koraku smo pregledali naslove in povzetke člankov ter izločili 820 člankov. Po tem pregledu smo nato pregledali vsebinsko še 96 člankov in jih 81 dodatno izključili. Končnih 15 zadetkov smo na koncu podrobno analizirali ter jih vključili v končno analizo. Na podlagi ugotovitev raziskav smo jih razporedili v štiri kategorije. Razprava: Diplomirane medicinske sestre imajo ključno vlogo pri zgodnjem odkrivanju septičnega šoka, saj so pogosto prve, ki opazijo simptome, vendar njihova uspešnost ni povsod optimalna. Znanje o sepsi in septičnem šoku je med sestrami zmerno, pri čemer le manjši delež pravilno opredeli stanje. Izkušnje, izobrazba in namensko financiranje izobraževanj izboljšujejo prepoznavanje, a v nekaterih okoljih primanjkuje poznavanja diagnostičnih orodij. Triažne medicinske sestre so pogosto zelo uspešne pri aktivaciji protokolov za zgodnjo obravnavo. Na uspešnost intervencij vplivajo strokovna usposobljenost, delovne izkušnje in skladnost s kliničnimi smernicami. Redna izobraževanja, zlasti simulacije, povečujejo samozavest in natančnost pri izvajanju ukrepov, prav tako imajo velik pomen izkušnje, saj izkušene sestre hitreje prepoznajo znake in učinkoviteje sledijo protokolom. Standardizirane klinične poti izboljšujejo skladnost z ukrepi in zmanjšujejo umrljivost, vendar je uspeh odvisen tudi od kadrovske organizacije. Preobremenjenost sester povečuje tveganje za napake in zamude, medtem ko ustrezno število osebja in dostop do diagnostičnih orodij, kot so hitri testi, izboljšujeta izide. Tehnološke rešitve in multidisciplinarni pristop dodatno podpirajo učinkovitost obravnave septičnega šoka.

Title:The role of the registered nurse in the management of the patient with septic shock
Abstract:Introduction: Sepsis and septic shock are urgent medical conditions that arise from an inadequate systemic response of the body to an infection, which can lead to organ failure and death. Septic shock is the most severe form of sepsis, characterized by a profound disruption in the functioning of organ systems due to the body's inadequate response to infection. A key role in the timely recognition and treatment of this condition is played by a graduate nurse. The graduate nurse has an important role in early recognition and management, as they monitor vital signs, administer fluids and antibiotics, use various assessment scales, and are often the first to notice early warning signs and changes. These may include changes in consciousness, hemodynamic and respiratory abnormalities, and laboratory indicators of inflammation. Methods: The research is based on a systematic literature review conducted in October 2024, utilizing the databases PubMed, Google Scholar, Cobiss, and ScienceDirect. A descriptive method was employed, involving a review of professional and scientific literature in Slovenian and English languages, related to the field of investigating the role of a graduate nurse in the management of a patient with septic shock. For the purpose of the research, freely accessible scientific-research and professional publication literature was used, covering the full scope and spanning the time period from January 2014 to October 2024. Results: During the literature review, based on a search strategy using keywords, we retrieved 916 hits from various scientific databases. In the next step, we reviewed the titles and abstracts of the articles and excluded 820 of them. Following this review, we conducted a content analysis of the remaining 96 articles and further excluded 81. The final 15 hits were included and thoroughly analyzed in the final analysis. Based on the research findings, we categorized them into four groups. Discussion: Registered nurses play a crucial role in the early detection of septic shock as they are often the first to notice symptoms, but their effectiveness is not optimal everywhere. Knowledge of sepsis and septic shock among nurses is moderate with only a smaller portion accurately identifying the condition. Experience, education, and targeted funding for training improve recognition though in some settings there is a lack of familiarity with diagnostic tools. Triage nurses are often highly successful in activating protocols for early intervention. The success of interventions depends on professional competence, work experience, and adherence to clinical guidelines. Regular training, especially simulations, boosts confidence and precision in implementing measures, while experienced nurses recognize signs more quickly and follow protocols more effectively. Standardized clinical pathways enhance compliance with measures and reduce mortality but success also hinges on staffing organization. Nurse overload increases the risk of errors and delays, whereas adequate staffing levels and access to diagnostic tools, such as rapid tests, improve outcomes. Technological solutions and a multidisciplinary approach further support the effectiveness of septic shock management.
