Title:Učinki fizioterapevtske obravnave v kombinaciji z neinvazivno stimulacijo možganov na motorične simptome parkinsonove bolezni : pregled literature

Abstract:Uvod: Parkinsonova bolezen, v nadaljevanju PB, je eno najpogostejših nevroloških obolenj na svetu, katerega pojavnost se, zaradi staranja prebivalstva, vsako leto povečuje (Osborne idr. 2022). Farmakološko zdravljenje v napredovalih fazah bolezni ni več tako učinkovito, zato se svetuje uporaba drugih oblik zdravljenja, kot sta fizioterapija in možganska stimulacija (Bueno idr. 2023). Pri osebah s PB se največkrat uporablja globoka možganska stimulacija, vendar so se v zadnjih letih pričele uporabljati tudi neinvazivne oblike stimulacije (Schulz idr. 2013; Larson 2014). Fizioterapija učinkovito vpliva na motorične simptome PB kot so bradikinezija, motnje hoje, ravnotežja in drže (Bouça-Machado idr. 2019). Namen: Namen diplomskega dela je bil raziskati, kako kombinirana terapija, ki vključuje fizioterapijo in neinvazivno možgansko stimulacijo vpliva na motorične simptome PB. Metode dela: Uporabili smo kvalitativno metodo raziskovanja s sistematičnim pregledom literature. Literaturo smo iskali s pomočjo podatkovnih baz: PubMed, Science Direct, Wiley Online Library, ResearchGate in Sage Journals. Rezultati: Pregled literature je zajemal 10 raziskav, objavljenih med leti 2013 in 2023. V štirih raziskavah so ugotovili učinkovitost kombinacije obeh metod, v dveh so ugotovili, da stimulacija podaljša učinke fizioterapije, brez znatnih izboljšanj merjenih parametrov. Ugotovitve štirih raziskav so pokazale, da kombinacija obeh metod ni povzročila izboljšanja pri ponovnih meritvah v primerjavi z vsako posamezno metodo. Zaključek: V izbranih raziskavah sta bili najpogosteje uporabljeni transkranialna magnetna stimulacija in stimulacija z električnim tokom. Kombinacija fizioterapije in stimulacije v določeni meri lahko vpliva na motorične simptome PB, vendar bi bile za statistično pomembne primerjave in ocene učinkovitosti metod v prihodnosti potrebne nadaljnje raziskave.

Abstract:Introduction: Parkinson's disease, hereafter PD, is a common neurological disease, with an increasing incidence due to ageing population (Osborne idr. 2022). Pharmacological treatment becomes less effective in later stages of PD, therefore other therapies like physiotherapy and brain stimulation are recommended (Bueno idr. 2023). Deep brain stimulation is the most widely used type of stimulation, but recently non-invasive techniques have come to use (Schulz idr. 2013; Larson 2014). Physiotherapy can positively effect motor symptoms of PD such as bradykinesia, gait, balance and posture disorders (Bouça-Machado idr. 2019). Purpose: The purpose of this thesis was to investigate the efficacy of combined therapy including physiotherapy and non-invasive brain stimulation on the motor symptoms of PD. Methods: We used a qualitative research approach with a systematic literature review. PubMed, Science Direct, Wiley Online Library, ResearchGate and Sage Journals databases were used for literature search. Results: Ten studies published between 2013 and 2023 were used in this literature review. Four studies found efficacy in the combination of both techniques, two studies found stimulation was effective only in prolonging the effects of physiotherapy, without a significant improvement of the measured outcomes. Four studies found the combination no more effective than individual therapies. Conclusion: Transcranial magnetic and direct current stimulation were most frequently used in the studies. The combination of physiotherapy and non-invasive brain stimulation can, to some extent, have an effect on the motor symptoms of PD, but statistically significant comparisons and assesments of the efficacy of methods in the future require further research.
