Title:Učinki fizioterapije na bolečino in funkcijo roke po operaciji karpalnega kanala : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Teoretična izhodišča: Sindrom karpalnega kanala je ena najpogostejših utesnitvenih nevropatij. Nastane zaradi utesnitve medianega živca na področju karpalnega kanala. Povzroča parestezije, bolečino ter v hujših oblikah atrofijo mišic. Zdravimo ga konservativno in operativno, fizioterapevtske metode se uporabljajo tako pri sindromu kot tudi po operaciji karpalnega kanala. Namen: Ugotoviti, katere fizioterapevtske metode so najbolj učinkovite pri izboljšanju funkcije roke in zmanjšanju bolečine po operaciji karpalnega kanala. Metode dela: Izvedli smo pregled strokovne in znanstvene literature. Za iskanje literature smo uporabili spletna orodja Google Scholar, PubMed, ScienceDirect, Cobiss+ in PEDro. Iskanje smo omejili na obdobje med letoma 2013 in 2023, članki so morali biti dostopni v polnem besedilu in napisani v angleškem ali slovenskem jeziku ter vključevati oceno bolečine in/ali funkcije roke. Rezultati: Raziskovalci v izbranih člankih so dokazali statistično pomemben vpliv (p < 0,05) globinskih udarnih valov, vaj, predpisanih preko računalniške tablice, ene terapije za roko ter vaj za drsenje živčevja na izboljšanje funkcije roke. Dokazali so statistično pomemben vpliv (p < 0,05) globinskih udarnih valov na zmanjšanje bolečine, kinezioterapije in terapije z ogledalom na zmanjšanje bolečine ponoči, terapije z ogledalom tudi bolečine v mirovanju. Razprava in zaključek: Ugotovili smo, da je na obe postavki hkrati najbolje učinkovala terapija z globinskimi udarnimi valovi. Najbolj učinkovite metode za zmanjšanje bolečine po operaciji karpalnega kanala so bile tudi terapija z ogledalom in kinezioterapija. Najbolj učinkovite metode pri izboljšanju funkcije roke pa vaje na računalniški tablici, vaje za drsenje živčevja in ena terapija za roko.

Abstract:Theoretical standpoints: Carpal tunnel syndrome is one of the most common entrapment neuropathies, caused by median nerve compression. It causes paresteshia, pain and muscle atrophy in severe cases. For treatment, conservative and operative techniques can be used. Physiotherapy is used to treat the syndrome itself or post carpal tunnel surgery. Purpose: Figure out, which physiotherapy methods are most effective for improving hand function and lowering pain post carpal tunnel surgery. Methods: We counducted a review of scientific and professional literature. Google Scholar, PubMed, ScienceDirect, Cobiss+ and PEDro online tools were used to find the literature. The articles had to be published between 2013 and 2023, written in english or slovene and include pain and/or hand function measures. Results: In our chosen articles, the researchers found statistically signifficant effect (p<0,05) of extracorporeal shock-wave therapy, exercises prescribed by a touch screen tablet, one hand therapy visit and nerve gliding exercises on hand function. They proved a statistically significant effect (p<0,05) of extracorporeal shock-wave therapy on lowering pain, kinesiotherapy and mirror therapy on lowering night pain, the latter also lowering pain at rest. Disscusion and conclusion: We have found that extracorporeal shock-wave therapy was the most effective in terms of improving hand function and lowering pain. Hand function was also greatly improved by exercises administered through a touchscreen tablet, nerve gliding exercises and one visit of hand therapy. Night pain was the best lowered by kinesiotherapy and mirror therapy, which also lowered pain at rest.
