Title:ZDRAVSTVENA NEGA PACIENTA PO OPERACIJI ZLOMA KOLKA

Abstract:Teoretična izhodišča: Zlom kolka je eden izmed najpogostejših poškodb pri starejših ljudeh, ki povzroči nenadno spremembo v življenju posameznika in ima resne posledice na izrazito poslabšanje kakovosti življenja ter je kljub napredku medicine še vedno lahko smrtno nevarna poškodba. Poškodba zahteva kompleksno bolnišnično zdravljenje, ki vključuje kirurški poseg, intenzivno pooperativno zdravstveno nego in ciljno usmerjeno rehabilitacijo. Vsak posameznik ima lahko po poškodbi zloma kolka težave z gibanjem in tako postane odvisen tudi od drugih oseb pri izvajanju življenjskih aktivnosti. Ustrezna oskrba pacienta po zlomu kolka, tako pred operacijo kot po njej, je ključna za uspešno zdravljenje poškodb, preprečevanje zapletov in zagotavljanje učinkovite rehabilitacije, s ciljem čim hitrejše in čim boljše povrnitve funkcionalnosti. Pooperativni nadzor vključuje natančne postopke in intervencije, katerih cilj je preprečiti morebitne zaplete in zagotoviti optimalno zdravljenje. Kontinuirano strokovno izpopolnjevanje je pomembno za učinkovito izvajanje navedenih nalog in prispeva k izboljšanju kakovosti oskrbe ter zadovoljstva pacienta po operativnem posegu zloma kolka. Namen diplomske naloge je bil preučiti zdravstveno nego pacienta po operativnem posegu zloma kolka. Metode: Raziskava je temeljila na kvalitativnem raziskovalnem pristopu in deskriptivni metodi dela. Kot inštrument za zbiranje podatkov smo izdelali predlogo za polstrukturiran intervju. Primarni podatki so bili pridobljeni s tehniko intervjuvanja. Vzorec je bil namenski in je v intervjuju vključeval tri diplomirane medicinske sestre in tri srednje medicinske sestre, zaposlene v zdravilišču na negovalnem oddelku za rehabilitacijo, ki imajo sedem let in več delovnih izkušenj na navedenem področju. Raziskava je potekala v mesecu januarju 2025. Rezultati: Za vse intervjuvanke pooperativni nadzor obsega pomen preprečevanja pooperativnih zapletov kot eno svojih temeljnih nalog, pri čemer posebej izpostavljajo skrb za higieno operativne rane, nadzor bolečine ter izvajanje ukrepov za preprečevanje venske tromboze, vse to pa predstavlja tudi ključne naloge za večino intervjuvank. Prehrana in hidracija ključno vplivata na hitrost celjenja in splošno moč pacienta. Vse sodelujoče so poudarile pomen zgodnje mobilizacije, ki jo pogosto prilagajajo glede na psihofizično stanje pacienta. Raziskava je tudi pokazala, da je individualiziran pristop nujen za uspešno rehabilitacijo. Medicinske sestre morajo dobro poznati fizične in psihološke značilnosti vsakega posameznika, saj lahko le s tem zagotovijo celostno obravnavo in zmanjšajo tveganje za ponovne poškodbe. Pomembno vlogo ima tudi čustvena podpora pacientom in svojcem, ki povečuje motivacijo za sodelovanje v rehabilitacijskem procesu. Razprava: Ker zlom kolka močno poslabša kakovost življenja pacienta, bi bilo priporočljivo, da se njegova bivalna okolica prilagodi njegovi varnosti, kakor tudi upošteva nasvete glede zdravil in njihovih učinkov. Ugotovitve raziskave, s podprto strokovno literaturo s tega področja, potrjujejo pomembnost večdimenzionalne vloge medicinske sestre pri celostni obravnavi pacienta po zlomu kolka, zlasti na področjih oskrbe rane, preprečevanja zapletov, prehrane, hidracije, mobilizacije ter individualne prilagoditve zdravstvene nege, saj kakovostna zdravstvena nega močno vpliva na izid zdravljenja, trajanje rehabilitacije in povrnitev pacienta v čim bolj samostojno življenje.

Title:NURSING CARE OF A PATIENT AFTER HIP FRACTURE SURGERY
Abstract:Background: Hip fractures are one of the most common injuries among older adults, causing a sudden change in a person´s life and leading to serious consequences that significantly worsen the quality of life. Despite advances in medicine, it remains a potentially lifethreatening injury that requires complex hospital treatment. This includes surgical intervention, intensive postoperative nursing care, and targeted rehabilitation. After a hip fracture, individuals often experience impaired mobility and become dependent on others for performing daily activities. Proper care before and after surgery is crucial for successful healing, the prevention of complications, and rehabilitation aimed at restoring the patient´s functionality as quickly and effectively as possible. Postoperative care involves precise procedures and interventions to avoid complications and ensure optimal treatment. Continuous professional development is essential fort he effective implementation of these tasks and contributes to improving the quality of care and patient satisfaction. The purpose of this thesis was to examine nursing care following hip fracture surgery. Methods: The research was based on a qualitative research approach and a descriptive method of work. A template for a semi-structured interview was developed as the instrument for data collection. The primary data were obtained through interviews. The sample was purposive and included three registered nurses and three nursing assistants working in a rehabilitation care department at a spa, all with seven or more years of work experience in the field. The research was conducted in January 2025. Results: For all the interviewees, postoperative monitoring includes the importance of preventing postoperative complications as one of its core tasks, with particular emphasis on the care of the surgical wound, pain management, and measures sto prevent venous thrombosis. These tasks are also key responsibilities for most of the interviewees. Nutrition and hydration significantly impact the speed of healing and the overall strenght of the patient. All participants highlighted the importance of early mobilization, which is often adjusted according to the patient´s physical and psychological condition. The research also showed that an individualized approach is essential for successful rehabilitation. Nurses must be wellversed in the physical and psychological characteristics of each individual, as only by doing so can they provide comprehensive care and reduce the risk of re-injury. Emotional support for patients and theis families also plays a crucial role in increasing motivation to participate in the rehabilitation process. Discussion: Since a hip fracture significantly worsens the patient´s quality of life, it is recommended that their living environment be adapted to ensure safety, as well as to take into account advice regarding medications and their effects. The findings of the research, supported by relevant professional literature in this field, confirm the importance of the multidimensional role of the nurse in the holistic care of patients after a hip fracture, particularly in areas such as wound care, complication prevention, nutrition, hydration, mobilization, and individualized adjustments to nursing care. High quality nursing care has a profound impact on treatment outcomes, the duration of rehabilitation, and the patient´s return to as independent a life as possible.
