Title:Učinkovitost terapije transkutane električne stimulacije živcev in udarnih globinskih valov na zmanjšanje bolečine pri bolnikih z osteoartritisom kolena – pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Kolenski sklep je zaradi svoje kompleksne anatomske strukture in funkcionalnih zahtev eden najbolj obremenjenih sklepov v človeškem telesu, zato je pogosto izpostavljen poškodbam ter obrabnim spremembam, kot je osteoartritis. Zdravljenje osteoartritisa kolena (OAK) sledi stopenjskemu pristopu, kjer nefarmakološki in farmakološki ukrepi predstavljajo temelj konzervativnega zdravljenja pred kirurškimi posegi. Med nefarmakološkimi metodami sta transkutana električna stimulacija živcev (TENS) in udarni globinski valovi (UGV) pomembna fizikalna agensa. Namen diplomskega dela je bil pregledati obstoječo literaturo, primerjati učinkovitost TENS in UGV terapije ter oceniti njun potencial za zmanjšanje bolečine pri pacientih z OAK. Uporabljen je bil sistematični pregled literature, ki je vključeval analizo randomiziranih kontroliranih raziskav, metaanaliz in sistematičnih pregledov na področju TENS in UGV pri zdravljenju OAK. Ugotovili smo, da so učinki TENS na bolečino različni. Nekatere raziskave poročajo o pozitivnih učinkih, medtem ko druge ne navajajo klinično pomembnih razlik v primerjavi s placebom. UGV se je izkazala kot obetavna metoda, ki lahko učinkovito zmanjša bolečino z učinki, ki trajajo do 12 mesecev. Učinkovitost UGV je odvisna od uporabljene gostote energijskega toka. Primerjava obeh metod nakazuje, da ima UGV večji potencial za učinkovito zmanjševanje bolečine, medtem ko je TENS enostavnejši za uporabo. Obe metodi predstavljata pomembni nefarmakološki možnosti zdravljenja, vendar je potrebna optimizacija protokolov zdravljenja in nadaljnje raziskave.

Abstract:The knee joint, due to its complex anatomical structure and functional requirements, is one of the most heavily loaded joints in the human body, making it frequently susceptible to injuries and degenerative changes, such as osteoarthritis. Treatment of knee osteoarthritis (KOA) follows a staged approach, where non-pharmacological and pharmacological interventions form the foundation of conservative treatment before surgical procedures. Among non-pharmacological methods, transcutaneous electrical nerve stimulation (TENS) and extracorporeal shock wave therapy (ESWT) are important physical agents. The aim of this thesis was to review existing literature, compare the effectiveness of TENS and ESWT therapy, and evaluate their potential for pain reduction in patients with KOA. A systematic literature review was conducted, which included analysis of randomized controlled trials, meta-analyses, and systematic reviews in the field of TENS and ESWT for treating KOA. We found that the effects of TENS on pain vary. Some studies report positive effects, while others do not indicate clinically significant differences compared to placebo. ESWT has proven to be a promising method that can effectively reduce pain with effects lasting up to 12 months. The effectiveness of ESWT depends on the energy flux density used. Comparison of both methods suggests that ESWT has greater potential for effective pain reduction, while TENS is simpler to use. Both methods represent important non-pharmacological treatment options, however, optimization of treatment protocols and further research is needed.
