Title:Fizioterapija po mikrofrakturiranju hrustanca kolenskega sklepa - pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Teoretična izhodišča: Poškodbe hrustanca kolenskega sklepa še danes povzročajo izziv v smislu zdravljenja oziroma regeneracije okvarjenega tkiva. Paciente s tovrstnimi poškodbami najbolj zaznamuje globoka bolečina znotraj kolena, katero lahko spremljajo še druge težave. Zdravljenje poškodb hrustanca je praviloma operativne narave, saj se hrustanec sam po sebi težko obnavlja. Operativna metoda mikrofrakturiranja predstavlja pogosto izbiro zdravljenja hrustanca predvsem zaradi razmeroma nizkih stroškov, enostavnega artroskopskega postopka in minimalne tehnične zahtevnosti. Fizioterapija po omenjenem posegu ima ključno vlogo pri povrnitvi funkcionalnosti kolenskega sklepa. Metoda: Metodologija diplomskega dela je kvantitativna. Za opredeljevanje ključnih pojmov smo v teoretičnem delu uporabili metode deskripcije, kompilacije in komparacije. V empiričnem delu so uporabljene metode analiziranja, deskripcije in komparacije izbranih raziskav. Podatki so zbrani in prikazani s pomočjo PRIZMA diagrama. Rezultati: Ugotovitve kažejo, da ne obstaja standardiziran rehabilitacijski protokol po mikrofrakturiranju hrustanca kolenskega sklepa. Variabilnost protokolov se kaže v smislu izbire in uporabe fizioterapevtskih metod ter njihovi časovni umestitvi v rehabilitacijski plan. Rezultati raziskav prikazujejo, da uporaba razbremenilne kolenske opornice srednjeročno poveča debelino regenerativnega hrustanca, medtem ko terapija pulznega elektromagnetnega polja pozitivno vpliva na klinične izide pri rehabilitaciji. Razprava: Po analizi rezultatov je ugotovljeno, da sta najpogosteje uporabljeni intervenciji v rehabilitacijskih protokolih razgibavanje s pasivno kinematično opornico in uporaba razbremenilne opornice za koleno. Slednja se je skupaj s terapijo pulznega elektromagnetnega polja izkazala za učinkovito metodo za izboljšanje funkcionalnosti kolena po posegu. Izboljšanje funkcionalnih parametrov lahko zasledimo do dveh let po operaciji, nakar se trend obrne v nasprotno smer, saj rezultati po petih letih kažejo znaten upad funkcionalnosti.

Abstract:Theoretical Background: Injuries to the cartilage of the knee joint continue to pose a challenge in terms of treatment and regeneration of the damaged tissue. Patients with such injuries are most notably affected by deep pain within the knee, which can be accompanied by other problems. The treatment of cartilage injuries is generally surgical in nature, as cartilage has a limited ability to regenerate on its own. The microfracture technique is a common choice for treating cartilage, primarily due to its relatively low cost, simple arthroscopic procedure, and minimal technical difficulty. Physiotherapy following this procedure plays a key role in restoring the functionality of the knee joint. Method: The methodology of this thesis is quantitative. To define the key concepts, we used methods of description, compilation, and comparison in the theoretical part. The empirical part employs methods of analysis, description, and comparison of selected studies. Data are collected and presented using the PRISMA diagram. Results: Findings indicate that there is no standardized rehabilitation protocol following microfracture surgery of the knee cartilage. Variability in protocols is evident in the choice and application of physiotherapeutic methods and their timing within the rehabilitation plan. Research results show that the use of an offloading knee brace increases the thickness of regenerative cartilage in the medium term, while pulsed electromagnetic field therapy positively affects clinical outcomes in rehabilitation. Discussion: After analyzing the results, it was found that the most commonly used interventions in rehabilitation protocols are passive range of motion exercises with a continuous passive motion machine and the use of an offloading knee brace. The latter, together with pulsed electromagnetic field therapy, has proven to be an effective method for improving knee functionality after surgery. Improvement in functional parameters can be observed up to two years post-operation, after which the trend reverses, as results after five years show a significant decline in functionality.
