Title:Učinek robotsko podprtega treninga hoje na hojo in ravnotežje pri osebah s parkinsonovo boleznijo – pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Teoretično izhodišče: Parkinsonova bolezen (PB) je kronična progresivna nevrodegenerativna motnja, katere natančnega vzroka še ne poznamo. Za bolezen je značilno pomanjkanje dopaminergičnih nevronov v bazalnem delu ganglijev, kar vodi do številnih motoričnih in nemotoričnih simptomov. PB je neozdravljiva. Možno je le lajšanje simptomov s konzervativnimi in operativnimi metodami. Z razvojem tehnologije postaja robotsko podprt trening hoje (RAGT) inovativen pristop k izboljševanju motenj hoje in ravnotežja pri PB. Namen: Namen diplomskega dela je bil opraviti pregled literature in ugotoviti učinke RAGT na hojo in ravnotežje pri osebah s PB. Metode: V diplomskem delu je bila uporabljena kvalitativna metoda. Raziskava je bila izvedena s pregledom literature (PRISMA diagramom) strokovno-znanstvenih člankov, objavljenih v letih 2014–2024. Ti so bili poiskani v podatkovnih bazah ScienceDirect, PubMed Central, PEDro, ResearchGate in Google Scholar. Rezultati: V pregled je bilo vključenih osem študij, ki smo jih izbrali na podlagi vključitvenih in izključitvenih kriterijev. Večina raziskav je poročala o pozitivnih učinkih RAGT na hitrost hoje, funkcionalno zmogljivost hoje pri vsakodnevnih aktivnostih, ravnotežje, premičnost in zmanjšanje »zamrznitvenih stanj« pri hoji pri osebah s PB. Na funkcionalno zmogljivost hoje ima pozitiven učinek tudi trening hoje na tekalni stezi (TT). Razprava: Študije so pokazale, da je RAGT učinkovita metoda za izboljšanje nekaterih parametrov hoje in ravnotežja. Pri izbiri RAGT je potrebno upoštevati posamezne potrebe bolnika in se posluževati kombiniranega pristopa pri izvedbi terapij. Potrebne bodo nadaljnje raziskave, ki bodo natančneje opredelile dolgoročni učinek in specifične cilje rehabilitacije z RAGT.

Abstract:Theoretical background: Parkinson's disease (PD) is a chronic progressive neurodegenerative disorder, the exact cause of which is not yet known. The disease is characterized by a lack of dopaminergic neurons in the basal ganglia, which leads to numerous motor and non-motor symptoms. PD is incurable. Symptoms relief is only possible with conservative and surgical methods. With the development of technology, robotic-assisted gait training (RAGT) is becoming an innovative approach for improving gait and balance disorders in PD. Purpose: The purpose of the thesis was to conduct a literature review and determine the effects of RAGT on gait and balance in people with PD. Methods: A qualitative method was used in the thesis. The research was conducted by reviewing the literature (PRISMA diagram) of professional and scientific articles published in the years 2014-2024. These were searched in the databases ScienceDirect, PubMed Central, PEDro, ResearchGate and Google Scholar. Results: Eight studies were included in the review, selected based on inclusion and exclusion criteria. Most studies reported positive effects of RAGT on walking speed, functional walking capacity in daily activities, balance, mobility and reduction of freezing states during walking in people with PD. Treadmill walking training (TT) also has a positive effect on functional walking capacity. Discussion: Studies have shown that RAGT is an effective method for improving some parameters of walking and balance. When choosing RAGT, it is necessary to take into account the individual needs of the patient and use a combined approach in the implementation of therapies. Further research will be needed to more precisely define the long-term effect and specific goals of rehabilitation with RAGT.
