Title:Prevalenca stresne urinske inkontinence in ozaveščenost o vadbi mišic medeničnega dna po vaginalnem porodu : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Nosečnost ter vaginalni porod v nekaterih primerih vodita do različnih disfunkcij medeničnega dna, med katerimi se pogosto pojavlja stresna urinska inkontinenca. Pojavnost stresne urinske inkontinence negativno vpliva na kakovost življenja žensk v poporodnem obdobju, zato je izvajanje vadbe mišic medeničnega dna ključnega pomena. Koncept vadbe temelji na izboljšanju mišične moči in vzdržljivosti, kar vodi do zmanjšane verjetnosti pojava in lajšanja simptomov stresne urinske inkontinence. Raziskava, izvedena v sklopu diplomskega dela, je temeljila na anketiranju 196 žensk, ki so ustrezale vnaprej določenim vključitvenim kriterijem. S statistično analizo podatkov smo ugotovili, da se je 73 % anketirank po porodu vsaj nekajkrat na mesec soočalo s stresno urinsko inkontinenco. Incidence uhajanja urina pa so ključno vplivale na nižjo kakovost življenja. Med drugim se je z višjo starostjo ob vaginalnem porodu, številom porodov ter poškodbo presredka med porodom, povečevala sama verjetnost pojava stresne urinske inkontinence, medtem ko večje stopnje stresne urinske inkontinence, opredeljene glede na pogostost simptomov, zaradi naštetih dejavnikov ni bilo mogoče dokazati. Največ anketirank (44 %) je informacije o vadbi mišic medeničnega dna pridobilo od fizioterapevta. 70 % anketirank je po porodu izvajalo vadbo mišic medeničnega dna. Na verjetnost izvajanja vadbe je vplivala tako stopnja izobrazbe kot tudi razumljivost in natančnost posredovanih informacij, kar poudarja ključno vlogo fizioterapevta pri natančni in kakovostni edukaciji žensk. Statistična analiza podatkov ni potrdila povezave med izvajanjem vadbe in zmanjšanjem incidenc uhajanja urina, vendar je deskriptivna analiza odgovorov pokazala, da je 65 % anketirank bilo mnenja, da je bila vadba koristna pri lajšanju simptomov stresne urinske inkontinence.

Abstract:Pregnancy and vaginal childbirth in some cases lead to various pelvic floor dysfunctions, including stress urinary incontinence. The occurrence of stress urinary incontinence has a negative impact on the quality of life of women in the postpartum period, and therefore pelvic floor muscle training during pregnancy and after birth is essential. The concept of exercise is based on improving muscle strength and endurance, leading to a reduced likelihood of occurrence and relief of symptoms of stress urinary incontinence. The research, carried out as part of the diploma thesis, was based on a survey of 196 women who met the inclusion criteria. Statistical analysis of the data showed that 73 % of the respondents experienced stress urinary incontinence at least several times a month after childbirth. Incidence of urine leakage was a key factor in the lower quality of life. It was found, among other things, that that with increasing age at vaginal delivery, higher number of births and perineal injury during childbirth, the likelihood of the occurrence of stress urinary incontinence increases, while a higher level of stress urinary incontinence, defined according to the frequency of symptoms could not be demonstrated due to these factors. Most respondents (44 %) obtained information on pelvic floor muscle training from a physiotherapist. 70 % of the respondents had exercised their pelvic floor muscles after childbirth. The likelihood of exercising was influenced by the level of education of the respondents, as well as the comprehensibility and accuracy of the information provided, which highlights the key role of the physiotherapist in providing accurate and quality education to women. Statistical analysis of the data did not confirm an association between pelvic floor muscle training and a reduction in the incidence of urinary leakage, but descriptive analysis of the responses showed that 65 % of respondents thought that pelvic floor muscle training was helpful in relieving symptoms of stress urinary incontinence.
